Description
Overview of the medicine
Brinzolamide is a carbonic anhydrase inhibitor used to lower elevated intraocular pressure (IOP) in patients with ocular hypertension or open-angle glaucoma. It works by reducing the production of aqueous humor.
Uses & Indications
Dosage
Adults
One drop in the affected eye(s) three times daily.
Elderly
Same as adults.
Renal_impairment
No specific dosage adjustment required for mild to moderate impairment. Not studied in severe impairment, caution advised.
How to Take
For ophthalmic use only. Instill one drop into the conjunctival sac. If more than one topical ophthalmic drug is being used, they should be administered at least 5 minutes apart.
Mechanism of Action
Brinzolamide inhibits carbonic anhydrase in the ciliary body of the eye. This enzyme is responsible for the formation of bicarbonate ions, which in turn leads to the production of aqueous humor. By inhibiting carbonic anhydrase, Brinzolamide decreases the rate of aqueous humor secretion, thereby lowering intraocular pressure.

Onset
Typically within 2 hours.
Excretion
Primarily via kidneys, mostly as unchanged drug and its N-desethyl metabolite.
Half life
Approximately 111 days in red blood cells due to binding to carbonic anhydrase. In plasma, much shorter.
Absorption
Absorbed systemically after ocular administration, but systemic levels are generally low.
Metabolism
Primarily metabolized by CYP3A4 to N-desethyl brinzolamide.
Side Effects
Contraindications
- •Hypersensitivity to the active substance or to any of the excipients.
- •Known hypersensitivity to sulfonamides.
- •Severe renal impairment.
- •Hyperchloraemic acidosis.
Drug Interactions
Salicylates (high dose)
May enhance the toxicity of carbonic anhydrase inhibitors.
Oral Carbonic Anhydrase Inhibitors
Potential for additive systemic effects, not recommended.
Storage
Store at room temperature (15-25°C), protect from light, do not freeze
Overdose
Systemic overdose is unlikely due to ocular administration. If swallowed, supportive measures are recommended. Electrolyte imbalance may occur.
Pregnancy & Lactation
Use only if the potential benefit justifies the potential risk to the fetus. Not known whether Brinzolamide is excreted in human milk. A risk to the suckling child cannot be excluded. Decision to discontinue breastfeeding or therapy should be made.
