Overview of the medicine
Broket 1 mg Tablet is an anxiolytic medication used for the short-term management of anxiety disorders and insomnia. It helps to calm the brain and nerves.
Uses & Indications
Dosage
Adults
General anxiety: 0.5-1 mg, 2-3 times daily. Insomnia: 1-2 mg at bedtime. Panic disorder: 1-2 mg, 2-3 times daily, may be increased up to 6 mg daily in divided doses.
Elderly
Initially 0.25-0.5 mg, 2-3 times daily. Titrate cautiously to minimize side effects.
Renal_impairment
No specific dosage adjustment guidance, use with caution. Consider lower doses if severe impairment.
How to Take
Take the tablet whole with water, with or without food. Do not chew or crush. Follow your doctor's instructions carefully.
Mechanism of Action
Broket selectively binds to specific sites on the GABA-A receptor complex, enhancing the inhibitory effects of GABA. This leads to hyperpolarization of neurons and reduced neuronal excitability, resulting in anxiolytic, sedative, and muscle relaxant effects.
Pharmacokinetics
Onset
Within 30-60 minutes after oral administration.
Excretion
Excreted mainly in the urine as metabolites, with a small amount in feces.
Half life
Approximately 10-20 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract; peak plasma concentrations reached within 1-2 hours.
Metabolism
Primarily metabolized in the liver via oxidative pathways, producing inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Broket or other GABA-A receptor modulators
- Acute narrow-angle glaucoma
- Severe respiratory insufficiency
- Severe hepatic impairment
- Sleep apnea syndrome
Drug Interactions
Cimetidine, Erythromycin, Ketoconazole
May inhibit Broket metabolism, leading to increased plasma levels and effects.
Alcohol and other CNS depressants (e.g., opioids, antidepressants, antihistamines)
Increased sedative effect and risk of respiratory depression.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, impaired coordination, diminished reflexes, and coma. Treatment is primarily supportive; maintain airway, monitor vital signs, and administer intravenous fluids if necessary. Flumazenil may be considered in severe cases, but caution is advised if the patient is on chronic Broket therapy due to precipitation of withdrawal.
Pregnancy & Lactation
Pregnancy Category D. Broket should be avoided during pregnancy due to potential fetal harm, including congenital malformations and withdrawal symptoms in neonates. It is excreted in breast milk and should not be used during lactation.
