Description
Overview of the medicine
Calac is a combination of Calcium Carbonate and Vitamin D3, used to prevent and treat calcium and vitamin D deficiencies. It plays a crucial role in maintaining strong bones and teeth, supporting muscle function, and nerve transmission.
Uses & Indications
Dosage
Adults
1-2 tablets daily, or as directed by the physician. It can be taken in a single dose or divided doses.
Elderly
Same as adults, but caution is advised in patients with impaired renal function.
Renal_impairment
Use with caution and monitor serum calcium levels. Dose adjustment may be necessary under medical supervision.
How to Take
Take the tablet orally with water, preferably with or immediately after meals to enhance absorption and reduce gastrointestinal upset.
Mechanism of Action
Calcium is an essential mineral required for bone formation, maintenance, and various metabolic processes. Vitamin D3 (Cholecalciferol) facilitates the absorption of calcium from the gastrointestinal tract and its deposition into bones. It also helps regulate calcium and phosphate homeostasis.
Pharmacokinetics
Onset
Therapeutic effects on bone density are gradual, over weeks to months of consistent use.
Excretion
Calcium is primarily excreted via kidneys (urine) and intestines (feces). Vitamin D metabolites are excreted mainly via bile and feces, with some renal excretion.
Half life
Calcium has variable elimination kinetics. Vitamin D3's half-life for its active metabolites is several weeks.
Absorption
Calcium carbonate is absorbed in the small intestine, with absorption enhanced by Vitamin D3. Vitamin D3 is absorbed in the small intestine and then hydroxylated in the liver and kidneys to its active form, calcitriol.
Metabolism
Calcium is not metabolized. Vitamin D3 is hydroxylated in the liver to 25-hydroxyvitamin D3 and then in the kidneys to 1,25-dihydroxyvitamin D3 (calcitriol), its active form.
Side Effects
Contraindications
- •Hypercalcemia (high calcium levels in blood)
- •Hypercalciuria (high calcium levels in urine)
- •Severe renal impairment
- •Known hypersensitivity to calcium carbonate or Vitamin D3
- •Hypervitaminosis D
Drug Interactions
Bisphosphonates
Calcium may interfere with absorption. Take Calac at a different time.
Corticosteroids
May reduce calcium absorption.
Iron preparations
Reduced absorption of both. Take at least 2 hours apart.
Cardiac glycosides
Increased risk of cardiac arrhythmias if hypercalcemia occurs.
Thiazide diuretics
May increase the risk of hypercalcemia.
Tetracyclines & Quinolone antibiotics
Calcium can chelate these, reducing their absorption. Administer at least 2 hours before or 4-6 hours after Calac.
Thyroid hormones (e.g., Levothyroxine)
Reduced absorption. Take Calac at least 4 hours apart.
Storage
Store below 30°C in a cool, dry place. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose (hypercalcemia) may include nausea, vomiting, constipation, thirst, polyuria, anorexia, muscle weakness, confusion, and in severe cases, cardiac arrhythmias and coma. Management involves discontinuing the product, hydration, and medical support. Monitor serum calcium levels.
Pregnancy & Lactation
Calac is often prescribed during pregnancy and lactation to meet increased calcium and vitamin D requirements. However, always consult with a healthcare provider before use to determine the appropriate dosage.
