Uses & Indications
Dosage
Adults
1-2 tablets daily, or as directed by a physician. Chewable tablets may be taken with or without food.
Elderly
Same as adult dosage, but caution advised for those with renal impairment.
Renal_impairment
Use with caution and monitor serum calcium and phosphate levels. Dosage adjustment may be necessary, and consultation with a physician is recommended.
How to Take
Oral administration. Chewable tablets should be chewed thoroughly before swallowing. Film-coated tablets can be swallowed whole with water. Best taken with food to enhance absorption and minimize gastrointestinal upset.
Mechanism of Action
Calcium Carbonate provides elemental calcium necessary for bone formation, muscle contraction, nerve function, and blood clotting. Vitamin D3 facilitates the absorption of calcium from the gut and its deposition in the bones, thereby regulating calcium and phosphate homeostasis.

Excretion

Absorption
Calcium absorption is primarily in the small intestine, enhanced by Vitamin D3. Vitamin D3 is absorbed from the small intestine and converted to active forms in the liver and kidneys.
Metabolism
Calcium is not metabolized. Vitamin D3 is hydroxylated in the liver to 25-hydroxyvitamin D (calcifediol) and then in the kidneys to 1,25-dihydroxyvitamin D (calcitriol), the active form.

Contraindications

Drug Interactions
Levothyroxine
Calcium may reduce levothyroxine absorption. Separate administration by at least 4 hours.
Corticosteroids
May reduce calcium absorption.
Cardiac glycosides
Hypercalcemia can potentiate the effects of cardiac glycosides, increasing the risk of arrhythmias.
Thiazide diuretics
May increase the risk of hypercalcemia by reducing calcium excretion.
Bisphosphonates, Tetracyclines, Fluoroquinolones, Iron preparations
Calcium can interfere with the absorption of these drugs. Administer calcium at least 2-3 hours before or after these medications.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcemia (nausea, vomiting, constipation, thirst, polyuria, muscle weakness, fatigue, confusion). In severe cases, cardiac arrhythmias and coma may occur. Management involves discontinuation of the supplement, hydration, and medical intervention as needed. Monitor serum calcium levels.
Pregnancy & Lactation
Calcium and Vitamin D are essential during pregnancy and lactation. This supplement can be used under medical supervision to meet increased demands and prevent deficiencies. Do not exceed recommended doses without consulting a physician.
