Description
Overview of the medicine
Prucalopride is a selective, high-affinity 5-HT4 receptor agonist used to treat chronic idiopathic constipation in adults. It works by stimulating colonic motility, facilitating bowel movements and improving stool consistency.
Uses & Indications
Dosage
Adults
The recommended dose is 2 mg once daily. For some patients, 1 mg once daily may be sufficient.
Elderly
For patients aged >65 years, initiate treatment with 1 mg once daily. If needed and tolerated, the dose can be increased to 2 mg once daily.
Renal_impairment
For patients with severe renal impairment (creatinine clearance < 30 mL/min), the dose is 1 mg once daily.
How to Take
Administer orally, with or without food, at any time of the day. Swallow the tablet whole with water.
Mechanism of Action
Prucalopride is a selective, high-affinity serotonin (5-HT4) receptor agonist. It stimulates 5-HT4 receptors in the enteric nervous system, leading to increased acetylcholine release and enhanced propulsive colonic motility. This effect helps restore normal gut function and promote bowel movements.

Approximately 24-48 hours for significant improvement in bowel movement frequency.
Excretion
Primarily excreted unchanged in urine (about 60-65% of the dose within 24 hours), with some fecal excretion.

Metabolism
Minimally metabolized by hepatic enzymes; primary metabolic pathways involve N-dealkylation and oxidation. No significant metabolism by CYP450 enzymes.
Side Effects
Contraindications
- Hypersensitivity to prucalopride or any of its excipients.
- Intestinal obstruction, inflammatory bowel disease (Crohn's disease, ulcerative colitis), toxic megacolon/megarectum, intestinal perforation.
Drug Interactions
Opioids
May reduce the prokinetic effect of prucalopride, as opioids can cause constipation. Consider higher doses of prucalopride if co-administered.
Ketoconazole, Erythromycin, Ciclosporin
May increase prucalopride plasma concentrations, potentially leading to increased side effects. Exercise caution.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
