Description
Overview of the medicine
Caltrol 0.25 mcg Capsule contains Calcitriol, which is the active form of Vitamin D3. It helps the body to absorb calcium and maintain proper levels of calcium and phosphate in the blood, essential for healthy bones and other functions.
Uses & Indications
Dosage
Adults
Initial dose 0.25 mcg once daily. Dosage may be increased by 0.25 mcg increments every 4-8 weeks based on serum calcium levels and clinical response. Maintenance dose typically ranges from 0.25-1 mcg daily.
Elderly
Similar to adult dosage, but close monitoring of serum calcium and phosphate is recommended due to potential age-related renal impairment.
Renal_impairment
Initial dose 0.25 mcg once daily or every other day. Dosage should be carefully titrated based on serum calcium levels, often requiring lower doses to avoid hypercalcemia.
How to Take
Take the capsule orally with or without food, usually once daily, as directed by your doctor. Swallow the capsule whole with water; do not crush, chew, or open it.
Mechanism of Action
Calcitriol binds to vitamin D receptors, enhancing intestinal absorption of calcium and phosphate, increasing renal reabsorption of calcium, and influencing calcium release from bone. It plays a crucial role in maintaining mineral homeostasis and bone health.
Pharmacokinetics
Onset
Onset of action is usually within 2-4 hours, with peak effects after 4-6 hours.
Excretion
Excreted mainly through the biliary route into feces, with a small amount excreted renally.
Half life
Plasma elimination half-life is approximately 5-8 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract.
Metabolism
Primarily metabolized in the liver by 24-hydroxylase enzyme.
Side Effects
Contraindications
- •Hypercalcemia (abnormally high levels of calcium in the blood).
- •Evidence of Vitamin D toxicity.
- •Hypersensitivity to Calcitriol or any of its components.
Drug Interactions
Corticosteroids
May counteract the effects of vitamin D.
Thiazide diuretics
Increased risk of hypercalcemia.
Barbiturates/Phenytoin
May increase calcitriol metabolism, reducing its effects.
Cardiac glycosides (e.g., Digoxin)
Increased risk of cardiac arrhythmias due to hypercalcemia.
Storage
Store in a cool, dry place, protected from light and moisture, at room temperature (below 30°C). Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcemia (nausea, vomiting, thirst, polyuria, constipation, weakness, muscle pain) and vitamin D toxicity. Management involves immediate discontinuation of the drug, hydration, and close monitoring of serum calcium. Severe cases may require hospitalization.
Pregnancy & Lactation
Pregnancy Category C. Calcitriol should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in breast milk; therefore, caution should be exercised when Calcitriol is administered to a nursing woman. Consult your doctor.
