Description
Overview of the medicine
Caltrol (Calcitriol) is a synthetic active form of Vitamin D3 (1,25-dihydroxycholecalciferol). It is used to treat hypocalcemia and metabolic bone disease in patients with chronic renal dialysis, hypoparathyroidism, and certain other conditions where the body cannot produce enough active vitamin D.
Uses & Indications
Dosage
Adults
Initial dose 0.5 mcg-1 mcg intravenously 3 times weekly. Adjusted based on serum calcium and PTH levels. Maintenance dose usually 0.5 mcg-4 mcg 3 times weekly.
Elderly
Similar to adults, but careful monitoring required due to potential for hypercalcemia.
Renal_impairment
Used specifically for chronic renal failure, dosage adjusted based on serum calcium, phosphate, and PTH levels. Careful monitoring is essential.
How to Take
Administered intravenously as a bolus injection. Do not mix with other solutions or drugs. Administer directly into the venous access during hemodialysis.
Mechanism of Action
Calcitriol is the active form of vitamin D3. It promotes the intestinal absorption of calcium and phosphorus, and regulates bone mineralization. It also acts on the parathyroid glands to suppress parathyroid hormone (PTH) secretion.
Pharmacokinetics
Onset
Within 2-6 hours.
Excretion
Mainly excreted in bile and feces, with some renal excretion.
Half life
Approximately 3-6 hours (variable).
Absorption
Rapidly absorbed after intravenous administration.
Metabolism
Primarily metabolized in the liver and kidney by 24-hydroxylase.
Side Effects
Contraindications
- •Hypercalcemia
- •Vitamin D toxicity
- •Hypersensitivity to calcitriol or any of its excipients
Drug Interactions
Phosphate binders
May require dosage adjustment to avoid hyperphosphatemia.
Thiazide diuretics
May increase the risk of hypercalcemia.
Digitalis glycosides
Increased risk of cardiac arrhythmias due to hypercalcemia.
Phenytoin, barbiturates
May accelerate calcitriol metabolism, requiring higher doses.
Magnesium-containing antacids
May lead to hypermagnesemia, especially in patients on dialysis.
Storage
Store at controlled room temperature (20°C to 25°C), protect from light. Do not freeze.
Overdose
Symptoms include hypercalcemia (anorexia, nausea, vomiting, polyuria, polydipsia, weakness, headache, stupor). Treatment involves immediate discontinuation, hydration, and possibly diuretics or corticosteroids. Closely monitor serum calcium levels.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. It is unknown if calcitriol is excreted in human milk; use with caution during lactation.
