Description
Overview of the medicine
Carbizol 10 mg Tablet is an antithyroid medication used to treat hyperthyroidism (overactive thyroid gland). It works by reducing the amount of thyroid hormones produced by the thyroid gland.
Uses & Indications
Dosage
Adults
Initial dose: 15-60 mg daily in 2-3 divided doses until euthyroid. Maintenance dose: 5-30 mg daily, usually as a single daily dose or in divided doses.
Elderly
Lower initial doses may be considered. Closely monitor thyroid function and adjust dose as needed.
Renal_impairment
Dose reduction may be necessary in severe renal impairment. Careful monitoring is advised.
How to Take
Take orally, with or without food, as directed by the physician. It can be taken as a single daily dose or in divided doses.
Mechanism of Action
Methimazole inhibits the synthesis of thyroid hormones by blocking the oxidation of iodine and the coupling of iodotyrosines (monoiodotyrosine and diiodotyrosine) in the thyroid gland, primarily by inhibiting the enzyme thyroid peroxidase.
Pharmacokinetics
Onset
Clinical effects may take several days to weeks to manifest.
Excretion
Primarily excreted in the urine, with a small amount excreted in feces.
Half life
Approximately 5-6 hours.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract.
Metabolism
Undergoes minimal hepatic metabolism.
Side Effects
Contraindications
- •Hypersensitivity to methimazole or other thionamides
- •Severe hepatic impairment
- •History of agranulocytosis or bone marrow depression due to methimazole or carbimazole
- •Pre-existing cholestasis
Drug Interactions
Digoxin
Serum digoxin levels may increase as thyroid function normalizes.
Theophylline
Theophylline clearance may decrease as thyroid function normalizes, leading to increased levels.
Beta-blockers
Dosage may need to be reduced as thyroid function normalizes, due to reduced adrenergic activity.
Anticoagulants (e.g., Warfarin)
Methimazole can enhance the anticoagulant effect by decreasing vitamin K-dependent clotting factors. Monitor INR closely.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, headache, fever, arthralgia, pruritus, and pancytopenia. Management involves gastric lavage and symptomatic and supportive treatment. Bone marrow suppression may necessitate granulocyte colony-stimulating factors.
Pregnancy & Lactation
Use with caution. Methimazole crosses the placenta and is secreted into breast milk. It should be used during pregnancy only if clearly needed, at the lowest effective dose. PTU may be preferred in the first trimester, while Methimazole is generally preferred in the second and third trimesters.
