Overview of the medicine
Carbizol 5 mg Tablet contains Carbimazole, an antithyroid drug used to treat hyperthyroidism by reducing the production of thyroid hormones.
Uses & Indications
Dosage
Adults
Initial dose: 15-60 mg daily, usually in divided doses. Maintenance dose: 5-15 mg daily.
Elderly
May require lower doses; monitor thyroid function closely.
Renal_impairment
Dose reduction may be necessary; monitor thyroid function closely.
How to Take
Oral administration. Take with or without food. Consistent timing of doses is recommended.
Mechanism of Action
Carbimazole is a pro-drug that is converted to its active metabolite, Methimazole. Methimazole inhibits the synthesis of thyroid hormones by preventing the incorporation of iodine into tyrosine residues on thyroglobulin and by inhibiting the coupling of iodotyrosines to form T3 and T4.

Onset
1-2 weeks for initial effect, 4-8 weeks for full therapeutic effect.
Excretion
Primarily excreted in the urine as methimazole and its inactive metabolites.
Half life
Carbimazole itself has a short half-life, but its active metabolite methimazole has a half-life of 6-9 hours (longer in hyperthyroid patients and renal impairment).
Absorption
Readily absorbed from the gastrointestinal tract. Peak plasma concentrations of methimazole occur 1-2 hours after oral administration of carbimazole.
Metabolism
Rapidly and completely metabolized to methimazole, the active metabolite.
Side Effects
Contraindications

Drug Interactions
Digoxin
Serum levels may increase as thyroid status normalizes.
Warfarin
May potentiate anticoagulant effect due to altered metabolism of clotting factors as thyroid status normalizes.
Theophylline
Clearance may decrease as thyroid status normalizes.
Beta-blockers
Dosage may need adjustment as thyroid status normalizes.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, epigastric distress, headache, fever, arthralgia, pruritus, and rarely agranulocytosis, pancytopenia, or hepatitis. Management is supportive, including gastric lavage or activated charcoal if recent. Monitor thyroid function and blood counts.
Pregnancy & Lactation
Pregnancy: Use with caution, lowest effective dose. Crosses the placenta and can cause fetal hypothyroidism and congenital malformations (e.g., aplasia cutis). Lactation: Excreted in breast milk; use with caution, monitor infant thyroid function.
