Description
Overview of the medicine
Cardisan Plus is a combination medication containing Losartan Potassium, an angiotensin receptor blocker (ARB), and Hydrochlorothiazide (HCTZ), a thiazide diuretic. It is primarily used to treat high blood pressure (hypertension), especially when a single agent is not sufficient to control blood pressure.
Uses & Indications
Dosage
Adults
Usually 1 tablet (Losartan 50 mg/HCTZ 12.5 mg) once daily. May be increased to 1 tablet (Losartan 100 mg/HCTZ 25 mg) once daily if blood pressure is not controlled.
Elderly
No initial dose adjustment is generally required, but close monitoring is advisable.
Renal_impairment
Not recommended for patients with severe renal impairment (creatinine clearance < 30 mL/min). Dose adjustment may be needed for moderate impairment.
How to Take
Orally, with or without food, preferably at the same time each day.
Mechanism of Action
Losartan blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin II by selectively blocking the binding of angiotensin II to the AT1 receptor. Hydrochlorothiazide increases the excretion of sodium, chloride, and water by inhibiting their reabsorption in the distal convoluted tubule.
Pharmacokinetics
Onset
Blood pressure lowering effect starts within 1 hour, peak effect within 3-6 hours.
Excretion
Losartan and its metabolites are excreted in urine (35%) and feces (60%). HCTZ is primarily excreted unchanged by the kidneys.
Half life
Losartan: 2 hours, active metabolite (E-3174): 6-9 hours. HCTZ: 5.6-14.8 hours.
Absorption
Losartan is well absorbed orally, with a bioavailability of about 33%. HCTZ is also well absorbed.
Metabolism
Losartan undergoes extensive first-pass metabolism to an active carboxylic acid metabolite (E-3174). HCTZ is not metabolized.
Side Effects
Contraindications
- •Hypersensitivity to Losartan, HCTZ, or sulfonamide-derived drugs
- •Anuria
- •Severe renal impairment
- •Severe hepatic impairment
- •Pregnancy (especially 2nd and 3rd trimesters)
- •Concomitant use with aliskiren in patients with diabetes or renal impairment
Drug Interactions
NSAIDs
May reduce the diuretic and antihypertensive effects of HCTZ and ARBs. May increase the risk of renal impairment.
Lithium
Renal clearance of lithium is reduced by diuretics and ARBs, increasing lithium toxicity.
Corticosteroids, ACTH
Increased electrolyte depletion, particularly hypokalemia.
Alcohol, barbiturates, narcotics
May potentiate orthostatic hypotension with HCTZ.
Potassium-sparing diuretics/Potassium supplements
May increase serum potassium levels, especially with Losartan.
Storage
Store below 30°C, in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms include hypotension, tachycardia, bradycardia (less common), and electrolyte depletion. Treatment is symptomatic and supportive; hemodialysis is unlikely to remove Losartan significantly, but may be useful for HCTZ.
Pregnancy & Lactation
Contraindicated during the 2nd and 3rd trimesters of pregnancy due to fetal toxicity. Not recommended during breastfeeding as both Losartan and HCTZ are excreted in breast milk.
