Description
Overview of the medicine
Aceclofenac is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain and inflammation in conditions like osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis.
Uses & Indications
Dosage
Adults
100 mg twice daily.
Elderly
Generally the same as adults, but caution is advised. The lowest effective dose should be used for the shortest possible duration.
Renal_impairment
No dose adjustment is usually required in mild renal impairment, but caution is advised. Avoid in severe renal impairment.
How to Take
Take orally, with or after food to minimize gastrointestinal upset. Swallow the tablet whole with water.
Mechanism of Action
Aceclofenac works by inhibiting the synthesis of prostaglandins, primarily through the inhibition of cyclooxygenase-2 (COX-2), which are chemicals that cause pain and inflammation.

Onset
Analgesic effects typically begin within 1 hour.
Excretion
Approximately two-thirds of the dose is excreted in the urine and one-third in the feces.
Half life
Approximately 4 hours.
Absorption
Well absorbed orally, with peak plasma concentrations reached in 1.25-3 hours.
Metabolism
Extensively metabolized in the liver, primarily via CYP2C9, to 4'-hydroxyaceclofenac and other metabolites.
Side Effects
Contraindications
- •Hypersensitivity to aceclofenac or any component of the formulation.
- •Patients with active gastrointestinal bleeding or ulceration.
- •History of asthma, urticaria, or other allergic-type reactions after taking aspirin or other NSAIDs.
- •Severe heart failure, severe renal impairment, or severe hepatic impairment.
- •Third trimester of pregnancy.
Drug Interactions
Lithium
Increased plasma lithium levels.
Methotrexate
Increased methotrexate toxicity.
Diuretics and ACE inhibitors
Reduced antihypertensive effect and increased risk of renal impairment.
Warfarin and other anticoagulants
Increased risk of bleeding.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include headache, nausea, vomiting, epigastric pain, gastrointestinal bleeding, and rarely, diarrhea, disorientation, excitation, coma, drowsiness, dizziness, tinnitus, and syncope. Management is symptomatic and supportive, including gastric lavage or activated charcoal.
Pregnancy & Lactation
Not recommended during the third trimester of pregnancy due to potential effects on fetal cardiovascular system. Use with caution in the first and second trimesters only if potential benefit outweighs risk. Avoid during lactation.
