Description
Overview of the medicine
Cefixime is a third-generation cephalosporin antibiotic used to treat various bacterial infections, including those of the respiratory tract, urinary tract, and ear infections. It is effective against a wide range of gram-positive and gram-negative bacteria.
Uses & Indications
Dosage
Adults
Not typically prescribed as a 90 mg suspension for adults. For general Cefixime, typical adult dose is 400 mg once daily or 200 mg every 12 hours.
Elderly
No specific dose adjustment is generally required unless there is severe renal impairment.
Children
8 mg/kg/day, given as a single daily dose or in two divided doses (e.g., 4 mg/kg every 12 hours). For this 90 mg/5 mL suspension, dosage should be calculated based on the child's weight and the concentration. Maximum daily dose should not exceed 400 mg.
Renal_impairment
Dosage adjustment is required for severe renal impairment (creatinine clearance < 30 mL/min). A dose of 200 mg once daily is recommended.
How to Take
Administer orally. The suspension should be shaken well before each use. It can be taken with or without food. Use a measuring spoon or oral syringe for accurate dosing. Complete the full prescribed course of medication.
Mechanism of Action
Cefixime is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the final transpeptidation step of peptidoglycan synthesis, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Peak plasma concentrations are generally achieved within 2-6 hours after oral administration.
Excretion
Primarily excreted unchanged in the urine (about 50%) and bile (about 10%) over 24 hours.
Half life
Approximately 3-4 hours.
Absorption
About 40-50% absorbed orally. Food may delay absorption but does not significantly affect the extent of absorption.
Metabolism
Cefixime is not significantly metabolized in the liver; some metabolites may be formed, but their contribution is minor.
Side Effects
Contraindications
- Hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reaction to penicillin or other beta-lactam antibiotics.
Drug Interactions
Warfarin
May increase prothrombin time (PT) and International Normalised Ratio (INR), increasing the risk of bleeding. Close monitoring is recommended.
Carbamazepine
Increased carbamazepine levels have been reported when co-administered with cefixime.
Oral Anticoagulants
Enhanced anticoagulant effect when co-administered with cefixime.
Storage
Store dry powder below 30°C, away from moisture and direct sunlight. Keep out of reach of children. Reconstituted suspension should be stored in a refrigerator (2-8°C) and discarded after 7-14 days depending on manufacturer instructions.
Overdose
In case of overdose, gastric lavage may be indicated. There is no specific antidote. Treatment should be supportive and symptomatic. Hemodialysis or peritoneal dialysis does not remove cefixime in significant quantities.
Pregnancy & Lactation
Pregnancy Category B. Animal reproduction studies have failed to demonstrate a risk to the fetus and there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Cefixime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.

Doctor Notes
- Counsel parents/guardians on the importance of completing the full course of antibiotics, even if symptoms improve, to prevent treatment failure and antibiotic resistance.
- Advise on proper reconstitution, storage, and accurate dosing of the suspension using the provided measuring device.
- Monitor patients for signs of allergic reactions, especially those with a history of penicillin allergy, and be prepared for anaphylaxis.
- Educate on potential side effects, particularly diarrhea, and advise to seek medical attention if severe or persistent.
Patient Guidelines
- Complete the full course of medication even if symptoms improve to prevent resistance and recurrence.
- Do not share this medication with others.
- Shake the suspension well before each use.
- Use the provided measuring device for accurate dosing and discard any unused portion after the specified period.
- Consult a doctor if symptoms do not improve or worsen.
Missed Dose Advice

Driving Precautions
Cefixime may cause dizziness or lightheadedness in some individuals. Patients should be advised to exercise caution when driving or operating machinery until they know how the medication affects them.
Lifestyle Advice
- Maintain good hydration, especially if experiencing diarrhea.
