Description
Overview of the medicine
Cefaclor is a second-generation cephalosporin antibiotic used to treat a wide range of bacterial infections including respiratory tract infections, skin and soft tissue infections, and urinary tract infections.
Uses & Indications
Dosage
Adults
250-500 mg orally every 8 hours. Max 4 g/day.
Elderly
Same as adults; caution in renal impairment, monitor kidney function.
Renal_impairment
Dose adjustment required based on creatinine clearance. For CrCl 10-50 mL/min: normal dose given less frequently. For CrCl < 10 mL/min: half dose given less frequently.
How to Take
Administer orally, with or without food. For better absorption, can be taken on an empty stomach, but if gastrointestinal upset occurs, take with food.
Mechanism of Action
Cefaclor is a bactericidal agent that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby inhibiting the third and final stage of bacterial cell wall synthesis.
Pharmacokinetics
Onset
Peak plasma concentrations are generally achieved within 0.5 to 1 hour.
Excretion
Mainly excreted by the kidneys (approximately 60-85% unchanged in urine within 8 hours).
Half life
Approximately 0.6 to 1 hour.
Absorption
Rapidly and well absorbed from the gastrointestinal tract after oral administration.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to cefaclor or other cephalosporins
- History of severe hypersensitivity reactions to penicillin (due to potential cross-reactivity)
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin and other oral anticoagulants.
Probenecid
Increases cefaclor plasma levels and prolongs its half-life by decreasing renal tubular secretion.
Storage
Store in a cool, dry place below 30°C, protected from light and moisture. Keep out of reach of children. Reconstituted suspension should be stored in a refrigerator (2-8°C) and used within 14 days.
Overdose
In case of overdose, seek immediate medical attention. Management should be supportive and symptomatic. Gastric lavage may be indicated if performed within 1-2 hours of ingestion. Hemodialysis is not substantially effective.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, use in pregnancy only if clearly needed. Excreted in breast milk in small amounts; use with caution during lactation.
