Cephid 125 mg Suspension contains Cephalexin, a first-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections susceptible to Cephalexin, especially in children, due to its palatable suspension form.
Uses & Indications
Dosage
Adults
Not typically used in suspension form for adults, but 250-1000 mg every 6-12 hours for adults, based on infection severity.
Elderly
Similar to adult dosage; however, renal function should be assessed and dosage adjusted if necessary.
Renal_impairment
Dosage adjustment is necessary in patients with impaired renal function (e.g., CrCl < 20 mL/min, interval extension or dose reduction).
How to Take
Administer orally. The suspension can be taken with or without food. For best absorption, it may be taken with food if gastrointestinal upset occurs. Shake well before each use. Complete the full prescribed course of treatment even if symptoms improve.
Mechanism of Action
Cephalexin, a bactericidal agent, inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). This disrupts the peptidoglycan layer, leading to osmotic lysis and bacterial cell death.
Pharmacokinetics
Onset
Therapeutic effects typically begin within an hour of administration.
Excretion
Primarily excreted unchanged in the urine by glomerular filtration and tubular secretion, with about 80% or more of the dose eliminated within 6 hours.
Half life
The elimination half-life is approximately 0.5-1.2 hours in individuals with normal renal function, increasing in renal impairment.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are usually achieved within 1-2 hours.
Metabolism
Cephalexin is not significantly metabolized in the liver.
Side Effects
Contraindications

Drug Interactions
Metformin
Cephalexin may increase the plasma concentration of metformin, requiring dose adjustment of metformin.
Probenecid
Concomitant administration of probenecid delays the renal excretion of cephalexin, leading to higher and more prolonged plasma concentrations.
Oral Anticoagulants
Rarely, cephalexin may enhance the anticoagulant effect of warfarin or other oral anticoagulants, increasing the risk of bleeding.
Storage
Store dry powder below 30°C, protected from light and moisture. Once reconstituted, store in a refrigerator (2-8°C) and discard any unused portion after 7 to 14 days (check specific product label).
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, and diarrhea. In case of massive overdose, general supportive measures are recommended, including gastric lavage or activated charcoal. Hemodialysis or peritoneal dialysis may be helpful.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cephalexin is excreted in human milk in low concentrations; caution should be exercised when administered to a nursing woman.
