Description
Overview of the medicine
Certican (Everolimus) is an immunosuppressant medication primarily used to prevent organ rejection in transplant recipients and to treat certain types of cancer, including advanced renal cell carcinoma, pancreatic neuroendocrine tumors, and subependymal giant cell astrocytoma.
Uses & Indications
Dosage
Adults
For transplant prophylaxis, typically 0.75 mg twice daily. For oncology indications, dosage varies significantly and should be determined by a specialist. Doses must be individualized based on therapeutic drug monitoring.
Elderly
No specific dose adjustment is generally required based on age, but patients should be monitored carefully for adverse reactions.
Renal_impairment
No dose adjustment is generally required in patients with renal impairment. However, everolimus blood levels should be monitored.
How to Take
Take Certican tablets orally, consistently either with or without food, at approximately the same time each day. Swallow the tablets whole with a glass of water; do not crush, chew, or break them.
Mechanism of Action
Everolimus is an inhibitor of mammalian target of rapamycin (mTOR), a serine/threonine kinase that plays a central role in regulating cell growth, proliferation, and survival. It forms a complex with FKBP-12, which then binds to and inhibits mTOR. This inhibition suppresses T-cell activation and proliferation, as well as antibody production, thereby reducing the immune response.
Pharmacokinetics
Onset
Immunosuppressive effects begin within hours to days of initiation.
Excretion
Primarily eliminated via feces (approximately 80%), with a minor amount (around 5%) excreted in urine.
Half life
Approximately 30-35 hours (variable, influenced by liver function and drug interactions).
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached within 1-2 hours. Absolute bioavailability is approximately 16%.
Metabolism
Extensively metabolized in the liver and intestinal wall via cytochrome P450 3A4 (CYP3A4).
Side Effects
Contraindications
- •Hypersensitivity to everolimus, sirolimus, or any of the excipients.
- •Patients who have previously experienced angioedema after taking an ACE inhibitor, or patients with hereditary angioedema.
Drug Interactions
Live vaccines
Avoid co-administration due to increased risk of infection.
ACE inhibitors
Increased risk of angioedema.
Other immunosuppressants (e.g., ciclosporin, tacrolimus)
Increased risk of nephrotoxicity and other adverse effects; careful monitoring required.
Strong CYP3A4 inducers (e.g., rifampicin, carbamazepine, phenytoin, St. John's wort)
Can significantly decrease everolimus blood levels, requiring dose increase.
Strong CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, clarithromycin, grapefruit juice)
Can significantly increase everolimus blood levels, requiring dose reduction.
Storage
Store below 30°C. Protect from moisture and light. Keep out of reach of children.
Overdose
In cases of overdose, general supportive measures should be initiated. Monitor blood everolimus levels and manage symptoms. There is no specific antidote.
Pregnancy & Lactation
Certican is not recommended during pregnancy due to potential fetal harm; women of childbearing potential should use highly effective contraception during treatment and for 8 weeks after the last dose. Avoid breastfeeding during treatment and for 2 weeks after the last dose.
