Overview of the medicine
Cetril 5 mg syrup contains cetirizine, a second-generation antihistamine used to relieve symptoms of various allergic conditions such as hay fever, perennial allergic rhinitis, and chronic urticaria.
Uses & Indications
Dosage
Adults
10 mg (10 ml) once daily or 5 mg (5 ml) twice daily.
Elderly
Start with 5 mg (5 ml) once daily, adjust based on renal function.
Renal_impairment
Dosage adjustment is necessary. For mild impairment, 5 mg once daily. For moderate to severe impairment (CrCl <30 ml/min), 5 mg every other day. Not recommended for severe renal impairment (CrCl <10 ml/min).
How to Take
Oral administration. May be taken with or without food. Use a measuring spoon or cup for accurate dosing.
Mechanism of Action
Cetirizine is a potent and selective antagonist of peripheral H1-receptors. It works by blocking the action of histamine, a substance in the body that causes allergic symptoms, thereby reducing allergic reactions like sneezing, runny nose, and itching.

Onset
Within 20-60 minutes.
Excretion
Primarily excreted unchanged in urine (about two-thirds of the dose) and to a lesser extent in feces.
Half life
Approximately 8-11 hours in adults.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations reached within 1 hour. Food may delay absorption but not the extent of absorption.
Metabolism
Minimal hepatic metabolism; primarily metabolized to an inactive carboxylic acid metabolite.
Side Effects
Contraindications
- •Known hypersensitivity to cetirizine, hydroxyzine, or any piperazine derivative.
- •Patients with severe renal impairment (creatinine clearance less than 10 ml/min).
Drug Interactions
Ritonavir
Increases cetirizine exposure (AUC) by about 40%, potentially requiring dosage adjustment of cetirizine.
Central Nervous System (CNS) Depressants
Concurrent use with alcohol or other CNS depressants may cause additive CNS depression (e.g., drowsiness).
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, agitation, restlessness, and rarely, convulsions. Management is symptomatic and supportive. There is no specific antidote for cetirizine. Gastric lavage may be considered soon after ingestion.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, use in pregnancy only if clearly needed and the potential benefits outweigh the potential risks. Cetirizine is excreted in breast milk; therefore, caution is advised when administered to a nursing woman.
