Description
Overview of the medicine
Cinnarizine is an antihistamine and calcium channel blocker used to treat motion sickness, vertigo, and Meniere's disease. It works by reducing the excitability of the vestibular system in the inner ear.
Uses & Indications
Dosage
Adults
For vertigo and motion sickness: 25-50 mg three times daily. For peripheral vascular disorders: 50-75 mg three times daily.
Elderly
Similar to adults, but use with caution due to increased risk of side effects, especially Parkinsonian symptoms. Start with a lower dose.
Renal_impairment
No specific dose adjustment is generally required, but caution is advised in severe impairment.
How to Take
Take orally with water, preferably after meals to minimize gastrointestinal upset. For motion sickness, take 30 minutes before travel.
Mechanism of Action
Cinnarizine primarily acts as an H1-receptor antagonist, blocking histamine receptors to reduce vestibular excitability. It also has a weak calcium channel blocking activity, particularly on vascular smooth muscle cells, leading to vasodilation and improved cerebral blood flow.
Pharmacokinetics
Onset
1-2 hours
Excretion
Excreted via urine (one-third) and feces (two-thirds), mainly as metabolites.
Half life
4-24 hours (highly variable)
Absorption
Well absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 1-3 hours.
Metabolism
Extensively metabolized by the liver, primarily via CYP2D6.
Side Effects
Contraindications
- •Hypersensitivity to cinnarizine or any component of the formulation
- •Parkinson's disease (may exacerbate symptoms)
- •Acute porphyria
Drug Interactions
Anticholinergic drugs
May have additive anticholinergic effects.
Antihypertensive agents
May potentiate the hypotensive effect.
CNS depressants (e.g., alcohol, sedatives, hypnotics)
May enhance sedative effects and impair psychomotor performance.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, stupor, ataxia, vomiting, and anticholinergic effects like dry mouth and blurred vision. Treatment is symptomatic and supportive, including gastric lavage and activated charcoal.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Cinnarizine may be excreted in breast milk; avoid use during lactation or discontinue breastfeeding.
