Description
Overview of the medicine
Cinaron Plus is a combination medicine containing Cinnarizine and Dimenhydrinate, used to treat symptoms of vertigo, dizziness, tinnitus, nausea, and vomiting associated with Meniere's disease and other vestibular disorders.
Uses & Indications
Dosage
Adults
One tablet two to three times daily. Maximum daily dose should not exceed 5 tablets.
Elderly
Same as adult dose, but caution is advised due to increased risk of side effects.
Renal_impairment
Use with caution. Dose adjustment might be necessary in severe renal impairment.
How to Take
To be taken orally, preferably after meals with some liquid, to reduce gastric irritation.
Mechanism of Action
Cinnarizine acts as a calcium channel blocker, inhibiting calcium influx into vestibular sensory cells, thereby reducing vestibular stimulation. It also has antihistaminic properties. Dimenhydrinate is an antihistamine (H1 receptor antagonist) with anticholinergic and antiemetic effects, primarily acting on the chemoreceptor trigger zone and vestibular nuclei.
Pharmacokinetics
Onset
Effects typically begin within 30-60 minutes after oral administration.
Excretion
Both drugs and their metabolites are primarily excreted in the urine.
Half life
Cinnarizine: 4-24 hours. Dimenhydrinate: 1-4 hours.
Absorption
Cinnarizine is slowly absorbed from the gastrointestinal tract, reaching peak plasma levels in 1-3 hours. Dimenhydrinate is well absorbed after oral administration, with effects usually noted within 30 minutes.
Metabolism
Both Cinnarizine and Dimenhydrinate are extensively metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to Cinnarizine, Dimenhydrinate, or other antihistamines
- Acute angle-closure glaucoma
- Urinary retention due to prostatic disorders
- Pyloroduodenal obstruction
- Epilepsy
- Children under 5 years of age
- Pregnancy and lactation (unless specifically advised by a physician)
Drug Interactions
Alcohol
Increased sedative effects.
Ototoxic drugs (e.g., aminoglycoside antibiotics)
May mask the warning signs of ototoxicity (e.g., tinnitus, dizziness).
CNS depressants (e.g., barbiturates, benzodiazepines, opioids)
Potentiated sedation and drowsiness.
Anticholinergic drugs (e.g., tricyclic antidepressants, atropine)
Increased anticholinergic effects (dry mouth, blurred vision, urinary retention).
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, dizziness, atropine-like effects (e.g., dry mouth, blurred vision, urinary retention), and in children, CNS excitation (e.g., convulsions, hallucinations). Management involves supportive care, gastric lavage, activated charcoal, and symptomatic treatment.
