Description
Overview of the medicine
Cinnarizine is an antihistamine primarily used to treat various conditions like vertigo, nausea, motion sickness, and symptoms associated with Meniere's disease. It helps to reduce dizziness and balance problems.
Uses & Indications
Dosage
Adults
For vertigo, Meniere's disease, or cerebral circulatory disorders: 1-2 tablets (10-20 mg) three times daily. For motion sickness: 2 tablets (20 mg) two hours before travel, then 1 tablet (10 mg) every 8 hours during travel.
Elderly
Elderly patients may be more sensitive to the effects; lower doses might be required. Monitor for extrapyramidal symptoms.
Renal_impairment
No specific dose adjustments are usually required for mild to moderate renal impairment. Use with caution in severe impairment; consult a physician.
How to Take
Take orally with or after meals to minimize gastrointestinal upset. Do not chew or crush the tablet.
Mechanism of Action
Cinnarizine acts as an H1-receptor antagonist, preventing histamine-induced vasodilatation and increased vascular permeability. It also inhibits calcium influx into smooth muscle cells, particularly in the vestibular system, thereby reducing the excitability of the vestibular apparatus.
Pharmacokinetics
Onset
Effects typically begin within 30 to 60 minutes after oral administration.
Excretion
Excreted via faeces (about 2/3) and urine (about 1/3) as metabolites.
Half life
Plasma elimination half-life is approximately 4 to 6 hours.
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma levels usually reached within 1 to 3 hours.
Metabolism
Extensively metabolized in the liver, primarily via CYP2D6.
Side Effects
Contraindications
- •Hypersensitivity to Cinnarizine or any component of the formulation
- •Acute porphyria
- •Parkinson's disease (may exacerbate symptoms)
- •Children under 5 years (for certain indications due to sedative effects)
Drug Interactions
Anticholinergic drugs
May result in additive anticholinergic effects.
Antihypertensive agents
May potentially enhance the hypotensive effects of antihypertensives.
CNS Depressants (e.g., alcohol, sedatives, hypnotics, opioid analgesics)
Concurrent use may enhance the sedative effects of Cinnarizine.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, stupor, coma, tremor, hypothermia, and extrapyramidal symptoms. Gastric lavage and activated charcoal may be considered. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Cinnarizine should be avoided during pregnancy unless the potential benefits outweigh the risks. Use with caution during lactation; consult a doctor as it is unknown if it passes into breast milk.
