Overview of the medicine
Cinekar is a medication containing Cinnarizine, a first-generation antihistamine that also acts as a calcium channel blocker. It is primarily used to treat symptoms of vertigo, dizziness, tinnitus, nausea and vomiting associated with Meniere's disease, and for the prevention of motion sickness.
Uses & Indications
Dosage
Adults
For vertigo and dizziness: 1-2 tablets (5-10 mg) three times daily. For motion sickness: 3 tablets (15 mg) two hours before travel, then 1-2 tablets (5-10 mg) every 8 hours during the journey.
Elderly
Similar to adult dosage, but caution should be exercised due to increased risk of sedation and extrapyramidal symptoms.
Renal_impairment
No specific dose adjustment is generally required, but caution is advised in severe impairment.
How to Take
Take orally, preferably after meals to minimize gastrointestinal irritation. Tablets can be swallowed whole with water.
Mechanism of Action
Cinnarizine acts as an H1-receptor antagonist, blocking histamine receptors in the vestibular system. It also inhibits calcium influx into vascular smooth muscle cells, reducing vasoconstriction and improving microcirculatory flow, particularly in the inner ear. This dual action contributes to its anti-vertigo and anti-emetic effects.

Onset
Anti-emetic and anti-vertigo effects may begin within 30 minutes to 1 hour.
Excretion
Approximately one-third of the metabolites are excreted in the urine and two-thirds in the feces.
Half life
Approximately 4 hours.
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma levels are achieved within 1 to 3 hours.
Metabolism
Extensively metabolized in the liver.
Side Effects
Contraindications

Tricyclic antidepressants

Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, sedation, ataxia (loss of coordination), hypotension, and in severe cases, coma. Treatment is symptomatic and supportive, including gastric lavage and activated charcoal if taken recently. Monitor vital signs.
Pregnancy & Lactation
Pregnancy Category C. Should only be used if the potential benefit outweighs the potential risk to the fetus. Cinnarizine may be excreted in breast milk; caution is advised during breastfeeding and it is recommended to consult a doctor.
