Overview of the medicine
Hiflox-XR (Ciprofloxacin Extended-Release) is a broad-spectrum fluoroquinolone antibiotic used to treat various bacterial infections, primarily urinary tract infections. Its extended-release formulation allows for once-daily dosing.
Uses & Indications
Dosage
Adults
Typically 500 mg or 1000 mg once daily, depending on the severity and type of infection. Treatment duration varies.
Elderly
Similar to adults, but careful monitoring of renal function is advised. Dose adjustment may be necessary based on creatinine clearance.
Renal_impairment
Dose adjustment required based on creatinine clearance. For CrCl 30-50 mL/min, 500 mg every 24 hours; for CrCl less than 30 mL/min, 250 mg every 24 hours.
How to Take
Take orally, preferably at the same time each day, with a glass of water. Do not crush, cut, or chew the tablet. It can be taken with or without food, but avoid taking it with dairy products or calcium-fortified juices as they can reduce absorption. Take at least 2 hours before or 6 hours after antacids containing magnesium or aluminum, or supplements containing iron or zinc.
Mechanism of Action
Ciprofloxacin works by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and cell death.

Onset
Within 1-2 hours of administration.
Excretion
Primarily excreted renally (approximately 50-70% as unchanged drug) and to a lesser extent via feces.
Half life
Approximately 4-5 hours (effective duration extended due to XR formulation).
Absorption
Well absorbed from the gastrointestinal tract; extended-release formulation provides sustained plasma concentrations over 24 hours.
Metabolism
Undergoes limited hepatic metabolism to less active metabolites.

Contraindications

Drug Interactions
Warfarin
Increased anticoagulant effect.
Tizanidine
Increased risk of hypotension and sedation.
Theophylline
Increased plasma levels of Theophylline, leading to toxicity.
Corticosteroids
Increased risk of tendon disorders.
Antacids (Magnesium/Aluminum), Iron/Zinc supplements
Reduced absorption of Ciprofloxacin.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptoms may include dizziness, tremor, headache, tiredness, seizures, abdominal pain, renal and hepatic impairment. Treatment should be symptomatic and supportive. Gastric lavage and activated charcoal may be considered if ingestion is recent.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Ciprofloxacin is excreted into breast milk; therefore, caution should be exercised when Hiflox-XR is administered to a nursing mother. Consult a doctor.
