Ciprofloxacin is a broad-spectrum antibiotic used to treat various bacterial infections, including those of the urinary tract, respiratory tract, skin, bone, and joint. It belongs to the fluoroquinolone class of antibiotics.
Uses & Indications
Dosage
Adults
Typical dose: 250 mg to 750 mg orally twice daily, depending on the infection type and severity. For most common infections, 500 mg twice daily is usual.
Elderly
No specific dose adjustment is needed based on age alone; adjust based on renal function.
Renal_impairment
Reduce dosage in patients with creatinine clearance < 50 mL/min. For CrCl 30-50 mL/min, 250-500 mg every 12 hours. For CrCl < 30 mL/min, 250-500 mg every 18-24 hours.
How to Take
Take orally with a glass of water, with or without food. Do not take with dairy products (milk, yogurt) or calcium-fortified juices alone, or with antacids containing magnesium or aluminum, as these can reduce absorption. Maintain adequate fluid intake.
Mechanism of Action
Ciprofloxacin works by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes necessary for bacterial DNA replication, transcription, repair, and recombination. This leads to bacterial cell death.

Pregnancy Category C. Ciprofloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk and may cause adverse effects in a nursing infant. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
