Ciprofloxacin is a broad-spectrum fluoroquinolone antibiotic used to treat various bacterial infections including urinary tract infections, respiratory tract infections, and skin infections.
Uses & Indications
Dosage
Adults
Typically 250-750 mg orally twice daily, depending on the severity and type of infection. Treatment duration varies.
Elderly
Dose adjustment may be necessary based on renal function. Generally, the lower end of the adult dosage range is preferred.
Children
Not generally recommended for routine use in children due to potential for arthropathy, consult physician. For specific severe infections (e.g., complicated UTI, anthrax exposure) 10-20 mg/kg orally twice daily (max 750 mg/dose).
Renal_impairment
For creatinine clearance < 50 mL/min, dose reduction is necessary. For example, for CrCl 30-50 mL/min, 250-500 mg every 12 hours; for CrCl < 30 mL/min, 250-500 mg every 18-24 hours.
How to Take
Shake the bottle well before each use. Take the measured dose orally with or without food. Do not chew the suspension. Ensure the full course is completed as prescribed.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes involved in bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.

Onset
Onset of action is variable, typically within 1-2 hours after oral administration.
Excretion
Primarily excreted renally (approximately 50-70% as unchanged drug in urine) and partly through the feces.
Half life
The elimination half-life is approximately 3-5 hours.
Absorption
Rapidly and well absorbed orally; bioavailability is approximately 70-80%. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Undergoes limited hepatic metabolism (10-20%) to several metabolites, some with minor antimicrobial activity.
Side Effects
Contraindications
- •Hypersensitivity to Ciprofloxacin, other quinolones, or any component of the formulation.
- •Concomitant administration with Tizanidine.
- •History of quinolone-associated tendinopathy.
Drug Interactions
Warfarin
Potentiates the anticoagulant effect of Warfarin, increasing bleeding risk. Monitor INR.
Theophylline
Ciprofloxacin can increase plasma levels of Theophylline, leading to toxicity. Monitor Theophylline levels.
Corticosteroids
Increased risk of tendinopathy and tendon rupture when co-administered with Ciprofloxacin.
Antacids/Dairy products/Mineral supplements
Reduce oral absorption of Ciprofloxacin. Administer Ciprofloxacin 2 hours before or 6 hours after these agents.
Storage
Store below 30°C, protected from light and moisture. Do not freeze. Keep out of reach of children. Discard any unused portion after the specified period.
Overdose
In case of overdose, gastric lavage may be performed. Symptomatic treatment and supportive care are recommended. Hydration should be maintained.
Pregnancy & Lactation
