Uses & Indications
Dosage
Adults
The usual intravenous dose for adults is 500 mg to 1 g every 8 hours, depending on the type and severity of infection. For certain mild to moderate infections or in specific cases, 250 mg may be considered, often in pediatric patients or those with renal impairment requiring dose adjustment.
Elderly
Dose adjustment based on renal function is generally recommended for elderly patients.

How to Take
Cispenam 250 mg Injection is for intravenous use only. It can be administered as an intravenous bolus over 3-5 minutes or as an intravenous infusion over 15-30 minutes. The specific route and duration depend on the dose and patient's condition.
Mechanism of Action
Meropenem (the active ingredient) is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). This leads to the disruption of the cell wall and subsequent bacterial cell death.

Side Effects
Contraindications
- Hypersensitivity to meropenem or any other carbapenem antibiotics.
- Patients with a history of severe hypersensitivity reactions (e.g., anaphylaxis, severe skin reactions) to any other beta-lactam antibiotic (e.g., penicillins, cephalosporins).
Drug Interactions
Probenecid
Probenecid competes with meropenem for active tubular secretion, leading to increased plasma concentrations and prolonged half-life of meropenem. Co-administration is not recommended.
Valproic acid
Concomitant administration with meropenem may decrease valproic acid levels below the therapeutic range, increasing the risk of seizures. Co-administration is generally not recommended.
Oral anticoagulants
Concurrent use with meropenem may enhance the anticoagulant effect of warfarin and other oral anticoagulants.
Storage
Store below 25°C. Protect from light and moisture. Reconstituted solutions should be used immediately or stored as per manufacturer's instructions, usually for a limited period under refrigeration.
Overdose
Limited experience with meropenem overdose. In the event of an overdose, discontinue meropenem and initiate general supportive treatment. Meropenem can be removed by hemodialysis.
