Description
Overview of the medicine
Cispenam 500 mg Injection is a broad-spectrum carbapenem antibiotic used to treat a wide range of bacterial infections, including severe and hospital-acquired infections. It is administered intravenously.
Uses & Indications
Dosage
Adults
Typically 500 mg to 1000 mg IV every 8 hours, depending on the severity and type of infection.
Elderly
Dose adjustments may be necessary based on renal function.
Renal_impairment
Dose reduction required for creatinine clearance <50 mL/min.
How to Take
For intravenous administration only. Can be given as a bolus injection over 5 minutes or as an intravenous infusion over 15-30 minutes.
Mechanism of Action
Meropenem is a carbapenem antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death. It has a broad spectrum of activity against gram-positive and gram-negative bacteria, including many anaerobic species.
Pharmacokinetics
Onset
Rapid, typically within minutes of IV administration.
Excretion
Primarily excreted unchanged in the urine (approximately 70% as unchanged drug).
Half life
Approximately 1 hour.
Absorption
Rapid and complete absorption after intravenous administration.
Metabolism
Minimally metabolized; primarily hydrolyzed by dehydropeptidase-I (DHP-I) in renal tubules, but meropenem is resistant to this enzyme.
Side Effects
Contraindications
- Hypersensitivity to meropenem, other carbapenems, or any component of the formulation
- Severe hypersensitivity to other beta-lactam antibacterial agents (e.g., penicillins, cephalosporins) due to potential cross-reactivity.
Drug Interactions
Probenecid
Increases meropenem plasma concentrations and prolongs its half-life by inhibiting renal tubular secretion.
Valproic acid
May decrease serum valproic acid concentrations, potentially leading to subtherapeutic levels and increased seizure risk. Co-administration is generally not recommended.
Storage
Store below 30°C. Protect from light. Do not freeze. Reconstituted solution should be used immediately.
Overdose
In case of overdose, discontinue the drug and initiate supportive treatment. Meropenem can be removed by hemodialysis.
Pregnancy & Lactation
Category B in pregnancy. Use only if clearly needed. Excreted in breast milk in small amounts; use with caution during lactation.
