Description
Overview of the medicine
Clobecort is a potent topical corticosteroid used to treat various inflammatory skin conditions such as psoriasis, eczema, and lichen planus, offering anti-inflammatory, antipruritic, and vasoconstrictive effects.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area once or twice daily. Treatment should be limited to 2-4 weeks. Do not use more than 50g per week.
Elderly
Use with caution. Similar dosage to adults, but consider increased skin fragility and potential for increased systemic absorption.
Renal_impairment
No specific dosage adjustment is usually required for topical application.
How to Take
For external use only. Apply a thin film to the affected skin and gently rub in. Do not apply to the face, groin, or axillae unless directed by a doctor. Wash hands thoroughly after application.
Mechanism of Action
Clobetasol propionate works by activating naturally occurring substances in the skin to reduce swelling, redness, and itching. It inhibits phospholipase A2, leading to decreased synthesis of inflammatory mediators like prostaglandins and leukotrienes.
Pharmacokinetics
Onset
Rapid, typically within a few hours to days.
Excretion
Excreted primarily by the kidneys, with some excretion in the bile.
Half life
Not well-defined for topical application; systemic corticosteroids have varied half-lives (e.g., prednisolone ~2-3 hours).
Absorption
Systemic absorption occurs, especially through damaged skin, large surface areas, or with occlusive dressings. Absorption is higher in areas with thin skin (e.g., face, genitalia).
Metabolism
Primarily metabolized in the liver, similar to other corticosteroids.
Side Effects
Contraindications
- Hypersensitivity to clobetasol propionate or any component of the formulation
- Untreated cutaneous infections (bacterial, fungal, viral, parasitic)
- Acne vulgaris, rosacea, perioral dermatitis
- Pruritus without inflammation
- Perianal and genital pruritus
Drug Interactions
0
CYP3A4 inhibitors (e.g., ritonavir, itraconazole) can increase systemic exposure to corticosteroids and lead to Cushing's syndrome and/or adrenal suppression. Use with caution.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep the tube tightly closed.
Overdose
Acute overdose is unlikely. Chronic overdose or misuse may lead to hypercorticism (Cushing's syndrome) and adrenal suppression. Treatment involves gradual withdrawal under medical supervision and symptomatic management.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown if topical clobetasol is excreted in human milk; use with caution in nursing mothers.
