Overview of the medicine
Clotinil contains Clopidogrel, an antiplatelet medication used to prevent blood clots in patients with heart disease, recent stroke, or peripheral arterial disease.
Uses & Indications
Dosage
Adults
Recent MI, recent stroke, or established PAD: 75 mg once daily. Acute Coronary Syndrome (Unstable Angina/NSTEMI): Loading dose of 300 mg or 600 mg, followed by 75 mg once daily (in combination with aspirin). STEMI (medically managed patients): 75 mg once daily (with aspirin), with or without a loading dose.
Elderly
No dose adjustment is generally required in elderly patients, unless specific renal or hepatic impairment is present.
Renal_impairment
Caution is advised. No specific dose adjustment guidelines are established, but monitoring for bleeding is recommended.
How to Take
Clotinil can be taken orally, with or without food. It is usually taken once daily.
Mechanism of Action
Clopidogrel selectively and irreversibly inhibits the binding of adenosine diphosphate (ADP) to its platelet P2Y12 receptor, thereby preventing ADP-mediated activation of the glycoprotein IIb/IIIa complex and subsequent platelet aggregation.
Pharmacokinetics
Onset
Antiplatelet effects begin within 2 hours; maximum effect achieved within 3-7 days with repeated dosing.
Excretion
Approximately 50% renally excreted and 46% fecally excreted within 120 hours after dosing.
Half life
Parent drug: Approximately 6 hours. Active metabolite: Approximately 8 hours.
Absorption
Rapidly absorbed after oral administration, but extensive first-pass metabolism results in low absolute bioavailability (approx. 50%).
Metabolism
Extensively metabolized in the liver, primarily by CYP450 enzymes (mainly CYP2C19, CYP3A4) to form an active thiol metabolite and inactive carboxylic acid derivatives.
Side Effects
Contraindications
- •Active pathological bleeding, such as peptic ulcer or intracranial hemorrhage.
- •Hypersensitivity to the active substance or to any of the excipients.
Drug Interactions
NSAIDs (e.g., ibuprofen, naproxen)
Increased risk of gastrointestinal bleeding. Use with caution.
Anticoagulants (e.g., warfarin, heparin)
Increased risk of bleeding. Close monitoring required.
Proton Pump Inhibitors (PPIs, e.g., omeprazole, esomeprazole)
May reduce the antiplatelet effect of clopidogrel due to inhibition of CYP2C19. Avoid concomitant use.
Other Antiplatelet agents (e.g., aspirin, prasugrel, ticagrelor)
Increased risk of bleeding.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Overdose may lead to increased bleeding complications. Management is symptomatic and supportive; platelet transfusions may be considered if rapid reversal of antiplatelet effect is required.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnancy only if clearly needed. Caution should be exercised when clopidogrel is administered to a nursing woman, as it is unknown if clopidogrel is excreted in human milk.
