Overview of the medicine
Cloxacillin is a narrow-spectrum beta-lactam antibiotic of the penicillin class. It is primarily used to treat infections caused by penicillinase-producing staphylococci, such as skin, soft tissue, bone, and respiratory tract infections.
Uses & Indications
Dosage
Adults
Intramuscular or Intravenous: 250-500 mg every 6 hours. For severe infections, up to 1 gram every 4-6 hours. Doses may vary based on infection severity and site.
Elderly
Similar to adult dosage, but caution and dose adjustment may be required in elderly patients with impaired renal function.
Renal_impairment
Dose adjustment required. For severe renal impairment (CrCl <10 mL/min), dosage may need to be reduced to 250-500 mg every 8-12 hours.
How to Take
Administer by deep intramuscular injection or by slow intravenous injection (over 3-4 minutes) or by intravenous infusion. Reconstitute with sterile water for injection according to package instructions.
Mechanism of Action
Cloxacillin is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs) located in the bacterial cell wall. This binding prevents the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.

Rapid, typically within minutes of intravenous administration.
Excretion
Primarily excreted unchanged in the urine via glomerular filtration and tubular secretion. A small amount is excreted in bile.
Half life
Approximately 0.5 to 1.5 hours in adults with normal renal function.
Absorption
Rapidly absorbed after intramuscular (IM) or intravenous (IV) administration. Peak serum concentrations are reached shortly after IV infusion.
Metabolism
Partially metabolized in the liver to active and inactive metabolites.
Side Effects
Contraindications

Drug Interactions
Warfarin
May alter anticoagulant effects; monitor INR closely.
Probenecid

Methotrexate
Penicillins can reduce the renal clearance of methotrexate, potentially increasing its toxicity.
Tetracyclines and Chloramphenicol
May antagonize the bactericidal effect of penicillinase-resistant penicillins.
Storage
Store dry powder vials at room temperature (below 30°C) and protect from moisture. Do not freeze. Store reconstituted solution as per manufacturer's guidelines, typically refrigerated.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and electrolyte imbalance. In rare cases, neurological effects (e.g., seizures) may occur with very high doses, particularly in patients with renal impairment. Management is symptomatic and supportive. Hemodialysis may be effective in removing cloxacillin from circulation.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown a risk, but human studies are not adequate. Use only if clearly needed. Cloxacillin is excreted in breast milk; use with caution in nursing mothers.
