Description
Overview of the medicine
Codcap 600 IU capsule is a vitamin D3 supplement used to prevent and treat vitamin D deficiency, supporting bone health, and boosting immunity.
Uses & Indications
Dosage
Adults
For vitamin D deficiency, usually 600-2000 IU daily, or as directed by a physician based on serum levels. For maintenance, 600 IU daily is often sufficient.
Elderly
Similar to adult dosage; higher doses may be required in some cases due to reduced skin synthesis and absorption. Consult a physician.
Renal_impairment
Use with caution. Monitor serum calcium and phosphate levels. Dosage adjustments may be necessary, and active vitamin D metabolites may be preferred in severe impairment.
How to Take
Take orally, preferably with a large meal containing fat to enhance absorption. Do not crush or chew the capsule; swallow whole with water.
Mechanism of Action
Cholecalciferol (Vitamin D3) is metabolized in the liver and kidneys into its active form, calcitriol, which helps regulate calcium and phosphate levels in the body by promoting intestinal absorption and bone mineralization.
Pharmacokinetics
Onset
The full therapeutic effect may take days to weeks to manifest as the body's vitamin D stores are replenished.
Excretion
Primarily excreted through the bile into the feces, with a small amount excreted in the urine.
Half life
The elimination half-life of cholecalciferol itself is approximately 15-30 days, but its active metabolites can have longer half-lives.
Absorption
Well absorbed from the gastrointestinal tract, especially when taken with fatty meals. Peak plasma concentrations are reached within 4-5 hours.
Metabolism
Initially hydroxylated in the liver to 25-hydroxycholecalciferol (calcifediol) and then in the kidneys to 1,25-dihydroxycholecalciferol (calcitriol), the active form.
Side Effects
Contraindications
- •Hypersensitivity to cholecalciferol or any component of the formulation
- •Hypercalcemia (high levels of calcium in the blood)
- •Hypervitaminosis D (excessive levels of vitamin D)
- •Malabsorption syndrome with severe vitamin D malabsorption
Drug Interactions
Corticosteroids
May reduce the effects of vitamin D by inhibiting its metabolism.
Thiazide diuretics
May increase the risk of hypercalcemia due to reduced calcium excretion.
Orlistat, Cholestyramine, Mineral oil
May reduce the absorption of vitamin D.
Anticonvulsants (e.g., phenytoin, carbamazepine)
May increase vitamin D metabolism, potentially leading to vitamin D deficiency.
Storage
Store below 30°C in a cool, dry place. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of vitamin D overdose (hypervitaminosis D) include hypercalcemia, which can lead to nausea, vomiting, constipation, polyuria, polydipsia, weakness, headache, and in severe cases, kidney damage or cardiac arrhythmias. Treatment involves discontinuing vitamin D, reducing calcium intake, and increasing fluid intake.
Pregnancy & Lactation
Vitamin D is generally safe and necessary during pregnancy and lactation at recommended daily allowances. Higher doses should only be used under medical supervision, with careful monitoring of maternal and infant calcium levels.
