Description
Overview of the medicine
Compiron 50 mg Pediatric Drop is an iron supplement used for the prevention and treatment of iron deficiency anemia in infants and children. It contains Iron Polymaltose Complex, which is well-tolerated and effectively absorbed, minimizing gastrointestinal side effects common with other iron forms.
Uses & Indications
Dosage
Adults
For children (1-12 years): Typically 2.5 mL (50 mg elemental iron) once daily, or as directed by a physician. Dosage depends on the severity of iron deficiency and individual requirements.
Elderly
For infants (0-1 year): Typically 1.25 mL (25 mg elemental iron) once daily. For prophylactic use in infants: 0.625-1.25 mL (12.5-25 mg elemental iron) once daily. This product is intended for pediatric use only, thus no specific dosage for adult or elderly patients is provided.
Renal_impairment
No specific dose adjustment required for pediatric patients with renal impairment, but caution is advised in severe cases due to potential iron accumulation. Monitor iron levels closely as advised by a physician.
How to Take
Administer orally using the calibrated dropper provided. May be mixed with fruit juice or water, but not with milk or antacids, as these may reduce iron absorption. Administer preferably between meals to maximize absorption, or with food if gastric upset occurs. For infants, drops can be given directly or mixed with a small amount of formula/breast milk just before feeding.
Mechanism of Action
Iron Polymaltose Complex provides elemental iron necessary for the synthesis of hemoglobin. Iron is a crucial component of hemoglobin, myoglobin, and various enzymes involved in oxygen transport and cellular respiration. The non-ionic nature of Iron Polymaltose Complex reduces interactions with food components and ensures controlled absorption.
Pharmacokinetics
Onset
Improvement in hemoglobin levels typically observed within 2-4 weeks of regular administration, with full therapeutic effect taking several months.
Excretion
Small amounts of iron are excreted via shedding of intestinal cells, bile, sweat, and urine. The body conserves iron efficiently, with minimal daily loss.
Half life
Iron in the body is complexed and stored; not applicable in a typical pharmacokinetic sense for elemental iron.
Absorption
Iron from Iron Polymaltose Complex is absorbed via a physiological mechanism, with a controlled and non-ionic uptake into the mucosal cells, largely independent of gastric pH. Absorption is not significantly affected by food components.
Metabolism
Iron is not metabolized in the traditional sense; it is incorporated into proteins like hemoglobin and ferritin or stored as hemosiderin.
Side Effects
Contraindications
- •Known hypersensitivity to Iron Polymaltose Complex or any excipients.
- •Iron overload conditions (e.g., hemochromatosis, hemosiderosis).
- •Anemias not caused by iron deficiency (e.g., hemolytic anemia, megaloblastic anemia, aplastic anemia).
- •Repeated blood transfusions, which may lead to iron overload.
- •Children with severe hepatic or renal dysfunction without careful medical supervision.
Drug Interactions
Antacids
May reduce iron absorption. Administer iron 2 hours before or 4 hours after antacids.
Quinolones
May reduce absorption of quinolones. Administer iron at least 2 hours before or 4-6 hours after quinolones.
Levothyroxine
May reduce levothyroxine absorption. Separate administration by at least 2-4 hours.
Tetracyclines
May reduce absorption of both iron and tetracycline. Separate administration by at least 2 hours.
Chloramphenicol
May delay the response to iron therapy.
Proton Pump Inhibitors (PPIs)
May reduce iron absorption due to reduced gastric acidity, although less pronounced with Iron Polymaltose Complex than with ferrous salts.
Storage
Store below 30°C in a dry place, protected from light and moisture. Do not freeze. Keep the bottle tightly closed after each use and keep out of reach of children.
Overdose
Symptoms of acute iron overdose in children include severe abdominal pain, vomiting, diarrhea, lethargy, pallor, and shock. In severe cases, metabolic acidosis, liver necrosis, and cardiovascular collapse can occur. Management involves gastric lavage, chelation therapy (e.g., deferoxamine) if indicated by serum iron levels, and supportive care. Seek immediate medical attention in case of suspected overdose.
Pregnancy & Lactation
This product is specifically for pediatric use. For pregnant or lactating women, iron supplementation is often necessary, but a physician should prescribe the appropriate formulation and dosage. Iron Polymaltose Complex is generally considered safe during these periods when prescribed by a doctor.
