Description
Overview of the medicine
Congenil 0.05% Nasal Drop is a topical decongestant used for the symptomatic relief of nasal congestion, perennial and allergic rhinitis, and sinusitis. It contains Xylometazoline Hydrochloride, an alpha-adrenergic agonist, which causes vasoconstriction of the nasal blood vessels, thereby reducing swelling and improving airflow.
Uses & Indications
Dosage
Adults
Not typically recommended for adults (0.1% strength is usually used). If 0.05% is prescribed, 1-2 drops into each nostril up to 3 times daily, for no more than 3-5 consecutive days.
Elderly
Same as adults, with caution. Consult a doctor.
Renal_impairment
No specific dose adjustment is generally required for topical nasal administration, as systemic absorption is minimal.
How to Take
For nasal use only. Tilt the head slightly back, insert the dropper into the nostril, and squeeze the required number of drops. Keep the head tilted for a few moments to allow the drops to spread. Avoid contact with eyes. Do not share the bottle with others to prevent infection.
Mechanism of Action
Xylometazoline acts as a direct-acting sympathomimetic amine. It stimulates alpha-adrenergic receptors in the arterioles of the nasal mucosa, causing vasoconstriction. This leads to decreased blood flow, reduced swelling of the nasal mucous membranes, and consequently, a reduction in nasal congestion.

Onset
Onset of action typically occurs within 5-10 minutes.
Excretion
Excretion of systemically absorbed drug and its metabolites occurs mainly via the kidneys (urine).
Half life
The systemic half-life, if absorbed, is approximately 5-10 hours, but local action is the primary effect.
Absorption
Minimal systemic absorption occurs when Xylometazoline is administered topically to the nasal mucosa. Any absorbed drug is rapidly and extensively distributed.
Metabolism
If systemically absorbed, it is primarily metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to Xylometazoline or any component of the formulation
- Patients with narrow-angle glaucoma
- Patients who have undergone transsphenoidal hypophysectomy or other surgery exposing the dura mater
- Patients with rhinitis sicca (dry inflammation of the nasal mucosa)
Drug Interactions
Tricyclic Antidepressants (TCAs)
May also enhance the cardiovascular effects of sympathomimetics.
Monoamine Oxidase Inhibitors (MAOIs)
Concurrent use may potentiate the pressor effect of Xylometazoline, leading to a hypertensive crisis. Avoid use within 14 days of discontinuing MAOIs.

Overdose
Overdose or prolonged excessive use can lead to systemic effects such as headache, tremors, insomnia, hypertension, and bradycardia. In children, CNS depression may occur. Management involves symptomatic and supportive treatment. Rebound congestion (rhinitis medicamentosa) is a common consequence of prolonged use.
