Overview of the medicine
Copixa 25 mg tablet contains Escitalopram, a selective serotonin reuptake inhibitor (SSRI) used to treat major depressive disorder, generalized anxiety disorder, and other psychiatric conditions.
Uses & Indications
Dosage
Adults
For depression/anxiety, usual starting dose is 10 mg once daily, increased to 20 mg after at least one week based on response. For Copixa 25 mg, the usual dose is one tablet once daily under strict medical supervision, as 25 mg is above the commonly recommended maximum for most indications.
Elderly
Usually a lower starting dose (e.g., 5 mg once daily), maximum 10 mg daily. 25 mg is generally not recommended for elderly patients.
Renal_impairment
No dosage adjustment is generally required for mild to moderate renal impairment. Use with caution in severe impairment.
How to Take
Oral administration. May be taken with or without food. Take at the same time each day.
Mechanism of Action
Escitalopram selectively inhibits the reuptake of serotonin (5-HT) in the central nervous system, thereby increasing the concentration of serotonin in the synaptic cleft and enhancing serotonergic neurotransmission.
Pharmacokinetics
Onset
Initial effects may be seen within 1-2 weeks; full antidepressant effect usually takes 2-4 weeks.
Excretion
Primarily through urine and feces as metabolites. Renal clearance is minor.
Half life
Approximately 27-32 hours, allowing for once-daily dosing.
Absorption
Well absorbed orally, bioavailability about 80%. Peak plasma concentrations achieved in 3-4 hours.
Metabolism
Extensively metabolized in the liver by CYP2C19, CYP3A4, and CYP2D6 to inactive or less active metabolites.
Side Effects
Contraindications
- •Hypersensitivity to escitalopram or citalopram
- •Concomitant use with MAOIs (Monoamine Oxidase Inhibitors) or within 14 days of stopping MAOI therapy
- •Concomitant use with pimozide
- •Congenital long QT syndrome or acquired QTc prolongation
- •Concomitant use with linezolid (an antibiotic with MAOI activity)
Drug Interactions
MAOIs
Risk of serotonin syndrome (hyperthermia, rigidity, myoclonus, autonomic instability, and rapid fluctuations of vital signs).
Pimozide
Increased QT prolongation and ventricular arrhythmias.
Warfarin
Potentiation of anticoagulant effect, leading to increased bleeding risk.
NSAIDs, Aspirin
Increased risk of gastrointestinal bleeding.
Tramadol, Triptans
Increased risk of serotonin syndrome.
Herbal remedies (e.g., St. John's Wort)
Increased serotonergic effects and risk of serotonin syndrome.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms include dizziness, tremor, nausea, vomiting, somnolence, sinus tachycardia, and QT prolongation. Management involves supportive care, maintaining airway, monitoring cardiac and vital signs. No specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Escitalopram is excreted in breast milk; advise caution as potential adverse effects on the infant cannot be excluded.
