Description
Overview of the medicine
Olanzapine is an atypical antipsychotic used to treat schizophrenia and bipolar disorder (manic or mixed episodes, and maintenance treatment). It helps to restore the balance of certain natural substances in the brain.
Uses & Indications
Dosage
Adults
Schizophrenia: Initially 10 mg once daily, adjusted to 5-20 mg/day based on clinical response. Bipolar Mania: Initially 10-15 mg once daily, adjusted to 5-20 mg/day. Maintenance: 5-20 mg once daily.
Elderly
Lower starting dose, e.g., 5 mg/day, with careful titration. Monitor for orthostatic hypotension.
Renal_impairment
No routine dosage adjustment is required, but caution is advised in severe cases. Start with a lower dose.
How to Take
Take orally, with or without food, usually once daily. It is recommended to take it at the same time each day.
Mechanism of Action
Olanzapine acts as an antagonist at multiple receptors including dopamine D1, D2, D4, serotonin 5-HT2A, 5-HT2C, 5-HT3, 5-HT6, muscarinic M1-M5, and adrenergic alpha1 receptors. This multifactorial action contributes to its efficacy in managing psychotic symptoms.
Pharmacokinetics
Onset
Clinical improvement typically observed within days to weeks of initiation.
Excretion
Approximately 57% excreted in urine and 30% in feces, mainly as metabolites.
Half life
21-54 hours (mean 33 hours).
Absorption
Well absorbed orally, peak plasma concentration reached in 5-8 hours. Food does not affect absorption.
Metabolism
Extensively metabolized in the liver, primarily by direct glucuronidation and by CYP1A2 and CYP2D6 pathways.
Side Effects
Contraindications
- •Hypersensitivity to Olanzapine or any component of the formulation.
- •Angle-closure glaucoma (due to its anticholinergic effects).
Drug Interactions
Anticholinergic Drugs
May increase anticholinergic side effects (e.g., dry mouth, constipation).
CYP1A2 Inhibitors (fluvoxamine)
May increase olanzapine levels, requiring dose reduction.
CNS Depressants (alcohol, benzodiazepines)
Increased CNS depressant effects.
CYP1A2 Inducers (carbamazepine, rifampicin)
May decrease olanzapine levels, potentially requiring dose increase.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms include somnolence, tachycardia, agitation, dysarthria, extrapyramidal symptoms, and coma. Treatment is supportive and symptomatic; there is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Olanzapine is excreted in breast milk; breastfeeding is not recommended due to potential adverse effects on the infant.
