Copnat-N 0.05% Cream is a combination medicine containing Clobetasol Propionate, a potent corticosteroid, and Neomycin Sulfate, an aminoglycoside antibiotic. It is used to treat inflammatory skin conditions such as eczema and psoriasis that are complicated by secondary bacterial infections.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area once or twice daily. Treatment should not exceed 2 consecutive weeks.
Elderly
Similar to adult dosage; use with caution due to thinner skin and potential for increased systemic absorption.
Renal_impairment
Not typically required for topical use, but caution is advised in cases of extensive skin application due to potential for systemic neomycin absorption and toxicity. Avoid prolonged use on large areas.
How to Take
For external use only. Wash hands before and after application. Apply a thin layer to the affected skin area and rub in gently. Do not cover with occlusive dressings unless specifically advised by a doctor. Avoid contact with eyes, nose, mouth, and other mucous membranes.
Mechanism of Action

Neomycin Sulfate is a broad-spectrum aminoglycoside antibiotic that inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, leading to bacterial cell death.

Excretion
Absorbed Clobetasol and its metabolites are excreted primarily by the kidneys, with some biliary excretion. Absorbed Neomycin is primarily excreted unchanged by the kidneys.

Systemic half-life of absorbed Clobetasol varies, generally 3-5 hours. Systemically absorbed Neomycin is rapidly excreted.
Absorption
Clobetasol propionate can be absorbed systemically from topical application, especially when applied to large areas, broken skin, or under occlusive dressings. Neomycin can also be absorbed systemically, particularly from denuded or traumatized skin, leading to potential systemic toxicity.
Metabolism
Clobetasol is primarily metabolized in the liver to inactive metabolites. Neomycin is not significantly metabolized.

Overdose
Acute overdose is unlikely with topical application. However, chronic excessive use or misuse (e.g., on large areas, under occlusive dressings, or for prolonged periods) can lead to systemic effects such as HPA axis suppression, Cushing's syndrome, hyperglycemia, and glucosuria due to clobetasol. Systemic neomycin absorption can lead to ototoxicity (hearing loss, vestibular damage) and nephrotoxicity (kidney damage). Treatment is symptomatic and supportive; discontinue the drug gradually.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Avoid large areas, prolonged use, or occlusive dressings. Exercise caution when administering to a nursing mother; it is not known whether topically applied corticosteroids are excreted in breast milk. Systemic absorption of neomycin may also occur.
