Overview of the medicine
Copnat-N 0.05% Ointment is a topical preparation containing Clobetasol Propionate, a potent corticosteroid, and Neomycin Sulfate, an aminoglycoside antibiotic. It is used to treat various inflammatory dermatoses where secondary bacterial infection is present or likely to occur.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area once or twice daily. Treatment should be limited to a maximum of 2 weeks. Do not use for more than 50g per week.
Elderly
Use with caution. Dosage adjustments may be necessary due to thinner skin and increased susceptibility to systemic absorption.
Renal_impairment
Systemic absorption of neomycin can be toxic; caution advised, especially with large or damaged skin areas. Consult a physician.
How to Take
For external use only. Apply a thin film to the affected skin area and rub in gently. Wash hands thoroughly before and after application. Do not cover with occlusive dressings unless advised by a doctor.
Mechanism of Action
Clobetasol Propionate acts as a potent corticosteroid, reducing inflammation, itching, and vasoconstriction. It achieves this by inducing phospholipase A2 inhibitory proteins, leading to a reduction in the biosynthesis of inflammatory mediators like prostaglandins and leukotrienes. Neomycin Sulfate is an aminoglycoside antibiotic that inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, thereby exhibiting bactericidal action against a wide range of gram-negative and some gram-positive bacteria.

Onset
Local anti-inflammatory and antibacterial effects typically within hours to days.
Excretion
Systemically absorbed Clobetasol and its metabolites are primarily excreted in urine and feces. Neomycin is primarily excreted unchanged in urine.
Half life
Systemic half-life of absorbed Clobetasol is around 3-5 hours; Neomycin's systemic half-life is 2-3 hours.
Absorption
Minimal systemic absorption through intact skin. Absorption increases with inflammation, skin barrier damage, or occlusive dressings. Neomycin absorption is also minimal but can increase with damaged skin.
Metabolism
Systemically absorbed Clobetasol is primarily metabolized in the liver. Neomycin is poorly metabolized.

Overdose

Neomycin systemic absorption from large areas of damaged skin may lead to ototoxicity or nephrotoxicity. Treatment is symptomatic and supportive; gradual withdrawal of the corticosteroid may be needed.
