Overview of the medicine
Coport M is an oral antidiabetic medication combining Glimepiride, a sulfonylurea, and Metformin Hydrochloride, a biguanide, used to improve glycemic control in adults with type 2 diabetes mellitus.
Uses & Indications
Dosage
Adults
Usually 1 tablet once or twice daily with meals. Initial dose might be Glimepiride 1-2 mg/Metformin 500 mg. Max dose Glimepiride 8 mg/Metformin 2000 mg daily.
Elderly
Start with a lower dose and titrate carefully due to potential for renal impairment.
Renal_impairment
Contraindicated in severe renal impairment (eGFR < 30 mL/min). Dose reduction may be needed in moderate impairment (eGFR 30-60 mL/min).
How to Take
Take orally with meals to reduce gastrointestinal side effects. Swallow whole, do not crush or chew.
Mechanism of Action
Glimepiride stimulates insulin release from pancreatic beta cells. Metformin reduces hepatic glucose production, decreases intestinal glucose absorption, and improves insulin sensitivity.

Glimepiride: 2-3 hours; Metformin: Several days for full effect.
Excretion
Glimepiride: Renally and fecally. Metformin: Primarily renal.
Half life
Glimepiride: 5-9 hours; Metformin: 4-9 hours.
Absorption
Glimepiride is completely absorbed from the GI tract. Metformin has low oral bioavailability (50-60%).
Metabolism
Glimepiride is extensively metabolized in the liver. Metformin is not metabolized and excreted unchanged.
Side Effects
Contraindications

- •Diabetic ketoacidosis
- •Severe renal impairment (eGFR < 30 mL/min)
- •Cardiac failure
- •Acute or chronic metabolic acidosis
- •Severe hepatic impairment
- •Acute myocardial infarction
- •Dehydration
Drug Interactions
Alcohol
Increases risk of lactic acidosis with Metformin and hypoglycemia with Glimepiride.
ACE Inhibitors, NSAIDs
May increase risk of renal impairment and lactic acidosis with Metformin.
Miconazole, Fluconazole
May potentiate the hypoglycemic effect of Glimepiride.
Iodinated contrast agents
Risk of acute renal failure and lactic acidosis with Metformin.
Corticosteroids, Diuretics
May reduce the hypoglycemic effect of Glimepiride and Metformin.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms may include severe hypoglycemia (with glimepiride), lactic acidosis (with metformin), nausea, vomiting, abdominal pain. Management includes supportive care, glucose administration for hypoglycemia, and hemodialysis for severe lactic acidosis.
Pregnancy & Lactation
Not recommended during pregnancy or lactation. Insulin is preferred for glycemic control during pregnancy.
