Description
Overview of the medicine
Cordef 24 mg tablet contains Deflazacort, a corticosteroid used to treat a wide range of inflammatory and autoimmune conditions, as well as Duchenne muscular dystrophy. It works by reducing inflammation and suppressing the immune system.
Uses & Indications
Dosage
Adults
Dosage varies greatly depending on the condition. For Duchenne Muscular Dystrophy, typically 0.9 mg/kg/day orally. For other conditions, initial dose may range from 6 mg to 18 mg per day, adjusted based on response. Cordef 24 mg is a higher strength, likely for severe conditions or specific loading doses.
Elderly
No specific dose adjustments required based on age alone, but caution with comorbidities. Use the lowest effective dose.
Renal_impairment
No specific dose adjustment recommended for mild to moderate renal impairment. For severe impairment, monitor for adverse effects.
How to Take
Take orally, with or without food. It is recommended to take it at the same time each day to maintain consistent drug levels. Do not stop suddenly without consulting a doctor due to risk of withdrawal symptoms.
Mechanism of Action
Deflazacort acts as a glucocorticoid, binding to specific intracellular glucocorticoid receptors. This complex then translocates into the nucleus, modulating gene expression. It inhibits the release of various inflammatory mediators (e.g., prostaglandins, leukotrienes) and suppresses the immune response by affecting lymphocyte function and migration.
Pharmacokinetics
Onset
Rapid, typically within hours for anti-inflammatory effects.
Excretion
Primarily excreted in the urine (approximately 70%) and feces (approximately 30%) as metabolites.
Half life
The active metabolite has a plasma half-life of approximately 1.5-2 hours.
Absorption
Well absorbed from the gastrointestinal tract, rapidly converted to its active metabolite (21-desacetyl deflazacort).
Metabolism
Rapidly metabolized in the liver to the active metabolite, 21-desacetyl deflazacort, which is then further metabolized.
Side Effects
Contraindications
- Hypersensitivity to Deflazacort or any component of the formulation
- Systemic fungal infections
- Administration of live or live attenuated vaccines in patients receiving immunosuppressive doses
Drug Interactions
Warfarin
May alter anticoagulant effect; monitor INR.
Live vaccines
Avoid during immunosuppressive therapy.
Ketoconazole, Erythromycin
May increase Deflazacort levels by inhibiting liver enzymes.
Diuretics (thiazide and loop)
Increased risk of hypokalemia.
Phenytoin, Phenobarbital, Rifampicin
May decrease Deflazacort levels by inducing liver enzymes.
Non-steroidal anti-inflammatory drugs (NSAIDs)
Increased risk of gastrointestinal bleeding.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is rare. Symptoms may include exaggeration of known side effects such as fluid retention, hyperglycemia, and adrenal suppression with chronic use. Management is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs risk to the fetus. Corticosteroids are excreted in breast milk; caution is advised during breastfeeding.
