Description
Overview of the medicine
Cosben is a multi-ingredient cough syrup indicated for the symptomatic relief of cough, cold, nasal congestion, and allergic conditions associated with respiratory tract infections. It combines a cough suppressant, an antihistamine, a decongestant, and an expectorant to provide comprehensive relief.
Uses & Indications
Dosage
Adults
10 ml (2 teaspoonfuls) 3-4 times a day.
Elderly
Same as adult dosage, but caution with comorbidities and polypharmacy. Consult doctor.
Children
6-12 years: 5 ml (1 teaspoonful) 3-4 times a day. Under 6 years: Not recommended or as directed by a physician.
Renal_impairment
Use with caution. Dosage adjustment may be required in severe cases. Consult doctor.
Hepatic_impairment
Use with caution. Dosage adjustment may be required in severe cases. Consult doctor.
How to Take
For oral administration only. Shake the bottle well before use. Use the provided measuring spoon or cup for accurate dosage.
Mechanism of Action
Dextromethorphan acts centrally on the cough center in the medulla to suppress cough. Diphenhydramine is an antihistamine that reduces allergic symptoms like sneezing and runny nose, and also has sedative properties. Phenylephrine is a decongestant that reduces nasal swelling by constricting blood vessels. Ammonium Chloride acts as an expectorant by irritating the bronchial mucosa, thinning and loosening phlegm.

Half life
Varies per component: Dextromethorphan (2-4 hours, up to 15 hours in poor metabolizers), Diphenhydramine (2-7 hours), Phenylephrine (2-3 hours), Ammonium Chloride (rapid).
Absorption
All active ingredients are readily absorbed from the gastrointestinal tract after oral administration.
Metabolism
Primarily hepatic metabolism for Dextromethorphan (CYP2D6), Diphenhydramine, and Phenylephrine.
Side Effects
Contraindications
- •Hypersensitivity to any of the active ingredients or excipients
- •Patients receiving monoamine oxidase inhibitors (MAOIs) or within 14 days of stopping MAOI therapy
- •Severe hypertension or severe coronary artery disease
- •Patients with severe liver or renal impairment
- •Children under 2 years (Dextromethorphan/Phenylephrine containing products are generally not recommended)
Drug Interactions
MAOIs
Concomitant use can lead to hypertensive crisis (with Phenylephrine) or serotonergic syndrome (with Dextromethorphan).
Antihypertensives
Phenylephrine may reduce the efficacy of antihypertensive drugs.
Other sympathomimetics
Increased risk of cardiovascular side effects.
CNS Depressants (alcohol, sedatives, tranquilizers)
Enhanced sedative effects with Diphenhydramine.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe drowsiness, dizziness, nausea, vomiting, rapid heartbeat, hypertension, hallucinations, and respiratory depression. In case of overdose, seek immediate medical attention. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Not recommended during pregnancy and lactation unless clearly necessary and the benefits outweigh the risks. Consult a doctor before use.
