Description
Overview of the medicine
Coughex is a combination medication used to relieve symptoms associated with cough, cold, and flu. It typically contains a cough suppressant (Dextromethorphan), an expectorant (Guaifenesin), and a nasal decongestant (Phenylephrine) to help clear airways and reduce coughing.
Uses & Indications
Dosage
Adults
10-20 mL orally every 4-6 hours as needed, not to exceed 80 mL in 24 hours
Elderly
Use with caution, often starting at lower doses due to potential increased sensitivity and comorbidities.
Renal_impairment
No specific dosage adjustment recommended for mild to moderate impairment, but use with caution. Severe impairment may require dose reduction.
How to Take
Oral use. Measure the dose with the provided measuring cup or spoon. May be taken with or without food.
Mechanism of Action
Dextromethorphan suppresses cough reflex by acting on the cough center in the medulla. Guaifenesin works by thinning and loosening mucus in the airways, making it easier to cough up. Phenylephrine is a decongestant that constricts blood vessels in the nasal passages, reducing swelling and congestion.
Pharmacokinetics
Onset
Within 15-30 minutes
Excretion
Renal
Half life
Dextromethorphan: 1.5-4 hours; Guaifenesin: approx. 1 hour; Phenylephrine: 2-3 hours
Absorption
Dextromethorphan is rapidly absorbed from the GI tract. Guaifenesin is well absorbed orally. Phenylephrine is well absorbed orally but has significant first-pass metabolism.
Metabolism
Dextromethorphan: Hepatic (CYP2D6). Guaifenesin: Rapidly metabolized in the liver. Phenylephrine: Extensively metabolized in the gut wall and liver (MAO, sulfotransferase).
