Description
Overview of the medicine
Coxitor 60 mg Tablet contains Etoricoxib, a selective COX-2 inhibitor, used for the relief of pain and inflammation associated with osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, and acute gouty arthritis. It is also used for the short-term treatment of moderate pain after dental surgery.
Uses & Indications
Dosage
Adults
For osteoarthritis: 30 mg or 60 mg once daily. For rheumatoid arthritis and ankylosing spondylitis: 60 mg or 90 mg once daily. For acute gouty arthritis: 120 mg once daily (maximum 8 days). For acute pain post-dental surgery: 90 mg once daily (maximum 3 days).
Elderly
No dosage adjustment is generally required, but caution is advised.
Renal_impairment
Mild to moderate renal impairment (creatinine clearance 30-80 mL/min): No dosage adjustment required. Severe renal impairment (creatinine clearance <30 mL/min): Use with caution, lower doses may be considered.
How to Take
Take orally, with or without food. For faster onset of action, it is recommended to take without food.
Mechanism of Action
Etoricoxib is a selective inhibitor of cyclooxygenase-2 (COX-2). COX-2 is responsible for the synthesis of prostaglandins, which are mediators of pain, inflammation, and fever. By inhibiting COX-2, Etoricoxib reduces prostaglandin production, thereby alleviating pain and inflammation.
Pharmacokinetics
Onset
Pain relief typically begins within 24 minutes to 1 hour after administration.
Excretion
Primarily excreted in the urine (about 70%) and feces (about 20%) as metabolites, with less than 1% excreted unchanged.
Half life
Approximately 22 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations (Cmax) reached in approximately 1 hour.
Metabolism
Extensively metabolized by the liver, primarily via cytochrome P450 (CYP3A4) to inactive metabolites.
Side Effects
Contraindications
- •Known hypersensitivity to etoricoxib or any component of the tablet
- •Active peptic ulceration or gastrointestinal bleeding
- •Severe hepatic dysfunction
- •Severe renal dysfunction (creatinine clearance <30 mL/min)
- •Congestive heart failure (NYHA Class II-IV)
- •Established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease
- •Pregnancy and lactation
Drug Interactions
Lithium
May increase plasma lithium levels, monitor lithium toxicity.
Warfarin
May increase the anticoagulant effect, leading to increased bleeding risk. Monitor INR.
Diuretics
May reduce the natriuretic effect of loop and thiazide diuretics.
Rifampicin
May decrease etoricoxib plasma concentrations due to enzyme induction.
Methotrexate
May increase plasma methotrexate concentrations, increasing toxicity. Use with caution.
ACE inhibitors/ARBs
May reduce the antihypertensive effect and increase the risk of renal impairment.
Oral Contraceptives
May increase plasma concentrations of ethinyl estradiol, potentially increasing adverse effects.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive measures should be initiated. There is no specific antidote for etoricoxib. Dialysis is unlikely to be effective in removing etoricoxib due to high protein binding.
Pregnancy & Lactation
Contraindicated during pregnancy and lactation. NSAIDs, including etoricoxib, can cause premature closure of the ductus arteriosus in the fetus in the third trimester and impair renal function. Avoid use while breastfeeding.
