Description
Overview of the medicine
Etoricoxib is a selective cyclooxygenase-2 (COX-2) inhibitor, belonging to the class of non-steroidal anti-inflammatory drugs (NSAIDs). It is used for the symptomatic relief of osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, and for the treatment of acute pain.
Uses & Indications
Dosage
Adults
Osteoarthritis: 30 mg or 60 mg once daily. Rheumatoid Arthritis/Ankylosing Spondylitis: 60 mg or 90 mg once daily. Acute Gouty Arthritis: 120 mg once daily for a maximum of 8 days. Acute Pain/Primary Dysmenorrhea: 60 mg or 90 mg once daily, not exceeding 120 mg for 8 days.
Elderly
No specific dosage adjustment is required based on age; however, caution should be exercised in elderly patients due to increased susceptibility to adverse effects, particularly cardiovascular.
Renal_impairment
Mild to moderate renal impairment (CrCl 30-80 mL/min): No dosage adjustment needed. Severe renal impairment (CrCl <30 mL/min): Not recommended.
How to Take
Administer orally, with or without food. Can be taken with food if gastrointestinal upset occurs.
Mechanism of Action
Etoricoxib selectively inhibits cyclooxygenase-2 (COX-2), which is responsible for the synthesis of prostaglandins involved in inflammation, pain, and fever. By inhibiting COX-2, it reduces inflammation and pain with a potentially lower risk of gastrointestinal side effects compared to non-selective NSAIDs.
Pharmacokinetics
Onset
For acute pain, onset of action can be within 24 minutes; for chronic conditions, therapeutic effects build over time.
Excretion
Eliminated predominantly by renal excretion (approximately 70% as metabolites) and fecal excretion (approximately 20% as metabolites).
Half life
Approximately 22 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations (Cmax) occurring approximately 1 hour post-dose.
Metabolism
Extensively metabolized by hepatic cytochrome P450 enzymes (mainly CYP3A4, also CYP2C9 and CYP2D6) to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to etoricoxib or any component
- •Active peptic ulceration or gastrointestinal (GI) bleeding
- •Severe hepatic dysfunction
- •Severe renal impairment (CrCl <30 mL/min)
- •Inflammatory bowel disease
- •Congestive heart failure (NYHA II-IV)
- •Established ischemic heart disease, peripheral arterial disease, and/or cerebrovascular disease
- •Uncontrolled hypertension
Drug Interactions
Lithium
Increased plasma lithium levels, potentially leading to toxicity.
Warfarin
Increased INR and risk of bleeding.
Methotrexate
Increased plasma methotrexate levels, potentially increasing toxicity.
Oral Contraceptives
Increased plasma concentration of oral contraceptives (estrogen component).
Diuretics, ACE Inhibitors, ARBs
Reduced antihypertensive effect and increased risk of renal impairment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Not recommended during the third trimester of pregnancy due to potential effects on fetal cardiovascular system. Avoid during lactation as it is excreted in breast milk.
