Description
Overview of the medicine
Cpzith-500-mg-tablet contains Azithromycin, a macrolide antibiotic used to treat various bacterial infections of the respiratory tract, skin, and genitourinary system.
Uses & Indications
Dosage
Adults
For most infections: 500 mg once daily for 3 days, or 500 mg on day 1 followed by 250 mg once daily for 4 days. For uncomplicated Chlamydia: A single 1g dose.
Elderly
No specific dose adjustment required based on age alone, but caution advised due to potential for age-related renal/hepatic impairment.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment (CrCl > 10 mL/min). Caution with severe renal impairment (CrCl < 10 mL/min).
How to Take
Should be taken orally with or without food. For optimal absorption and tolerability, it can be taken 1 hour before or 2 hours after food.
Mechanism of Action
Azithromycin binds to the 50S ribosomal subunit of susceptible microorganisms, thereby inhibiting bacterial protein synthesis and impeding their growth and replication.
Pharmacokinetics
Onset
Clinical effects typically observed within 1-2 hours after administration.
Excretion
Mainly excreted unchanged in bile; a small amount is excreted in urine (about 6% of the dose).
Half life
Terminal elimination half-life is 2-4 days (40-68 hours) due to extensive tissue uptake and slow release from tissues.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37%. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
Primarily hepatic, but minimal metabolism in the liver; main elimination is through biliary excretion.
Side Effects
Contraindications
- Hypersensitivity to azithromycin, erythromycin, or any macrolide or ketolide antibiotic
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use
Drug Interactions
Digoxin
May increase serum digoxin levels; monitor digoxin levels.
Warfarin
May enhance the anticoagulant effect of warfarin; monitor INR closely.
Ergot alkaloids (e.g., Ergotamine)
Potential for acute ergot toxicity (vasospasm with ischemia); concomitant use is contraindicated.
Antacids (containing aluminum or magnesium)
Decrease peak plasma concentrations of azithromycin; administer azithromycin at least 1 hour before or 2 hours after antacids.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe nausea, vomiting, diarrhea, and reversible hearing loss. Treatment is symptomatic and supportive; gastric lavage and general supportive measures may be indicated.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use only if clearly needed. Azithromycin is excreted in breast milk; caution should be exercised when administered to a nursing woman.

YesClinical Trials
Azithromycin has been extensively studied in numerous clinical trials demonstrating its efficacy and safety across a wide range of bacterial infections. Ongoing research explores new indications and resistance patterns.
Lab Monitoring
- Liver function tests (LFTs) if prolonged use or pre-existing hepatic impairment is suspected.
- Renal function tests if severe renal impairment (CrCl < 10 mL/min) is present.
- Electrolytes (especially potassium and magnesium) if risk of QT prolongation is high.
Doctor Notes
- Emphasize the importance of completing the full course of treatment to prevent antibiotic resistance and ensure efficacy.
- Advise patients to report any severe or persistent diarrhea, as it could indicate C. difficile infection.
- Be mindful of potential QT prolongation, especially in patients with pre-existing cardiac conditions, hypokalemia, or those on other QT-prolonging drugs.
- Consider liver function tests in patients with hepatic impairment or during prolonged therapy.
Patient Guidelines
- Complete the full course of treatment as prescribed by your doctor, even if symptoms improve.
- Do not share this medicine with others, even if they have similar symptoms.
- Inform your doctor or pharmacist about all other medications you are taking, including over-the-counter drugs and herbal supplements.
- Report any severe or persistent diarrhea, or signs of allergic reaction (rash, swelling) to your doctor immediately.
Missed Dose Advice
If a dose is missed, take it as soon as remembered unless it is almost time for the next dose. In that case, skip the missed dose and continue with the regular schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Azithromycin may cause dizziness or visual disturbances in some patients. Patients should be cautious when driving or operating machinery until they know how the medicine affects them.
Lifestyle Advice
- Maintain good hydration during treatment by drinking plenty of fluids.
- Avoid alcohol consumption as it may worsen stomach upset and other side effects.
