Description
Overview of the medicine
Cytapen-V 125 mg Suspension is an antibiotic from the penicillin group, used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
Typically 250-500 mg every 6-8 hours. For 125 mg/5ml suspension, 10-20 ml (250-500 mg) every 6-8 hours.
Elderly
Same as adults, but monitor renal function and adjust dose if severe impairment is present.
Renal_impairment
Dosage adjustment may be required in severe renal impairment (CrCl < 10 ml/min). Consult physician.
How to Take
For oral use. Can be taken with or without food. Shake the bottle well before each use. Use the provided measuring device to ensure accurate dosing. Complete the full prescribed course of medication.
Mechanism of Action
Phenoxymethylpenicillin, a beta-lactam antibiotic, inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid, typically within 30-60 minutes.
Excretion
Primarily excreted unchanged in the urine by tubular secretion, with some biliary excretion.
Half life
Approximately 30-60 minutes.
Absorption
Well absorbed from the gastrointestinal tract, stable in gastric acid. Peak plasma concentrations are reached within 30-60 minutes.
Metabolism
Partially metabolized in the liver (10-30%) to inactive metabolites.
Side Effects
Contraindications
- •Known hypersensitivity to penicillin or other beta-lactam antibiotics (e.g., cephalosporins).
- •Severe gastrointestinal disease that may impair absorption.
- •Infectious mononucleosis (increased risk of rash).
Drug Interactions
Probenecid
Decreases the renal tubular secretion of penicillin, leading to increased and prolonged blood levels.
Methotrexate
May increase methotrexate toxicity due to reduced renal clearance.
Tetracyclines
May antagonize the bactericidal effect of penicillin.
Oral Contraceptives
May reduce the efficacy of oral contraceptives (though clinical significance is debated).
Storage
Store unopened bottle below 30°C, protected from light and moisture. After reconstitution, store in a refrigerator (2-8°C). Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and rarely, convulsions, especially in patients with impaired renal function. Treatment is symptomatic and supportive, including gastric lavage or activated charcoal if indicated.
Pregnancy & Lactation
Pregnancy Category B. Phenoxymethylpenicillin is generally considered safe during pregnancy. It is excreted in breast milk in small amounts; generally considered safe during lactation, but monitor infant for potential side effects (e.g., diarrhea, candidiasis).
