Description
Overview of the medicine
Cytapen-V 250 mg Tablet is an antibiotic belonging to the penicillin group. It is used to treat a variety of bacterial infections like those of the throat, ear, nose, skin, and soft tissue.
Uses & Indications
Dosage
Adults
250-500 mg every 6-8 hours for 7-10 days depending on the infection severity and type.
Elderly
Similar to adult dosage, but caution with renal impairment.
Renal_impairment
Dosage adjustment may be necessary for severe renal impairment (e.g., CrCl <10 mL/min, reduce dose or extend interval).
How to Take
Take orally with water, preferably on an empty stomach (1 hour before or 2 hours after meals) for optimal absorption. May be taken with food to reduce gastrointestinal upset.
Mechanism of Action
Phenoxymethylpenicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 30-60 minutes after oral administration.
Excretion
Primarily excreted unchanged by the kidneys via tubular secretion.
Half life
Approximately 30-60 minutes in individuals with normal renal function.
Absorption
Well absorbed from the gastrointestinal tract (about 60-70% bioavailability).
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to penicillin or any component of the formulation.
- •Severe allergic reactions to other beta-lactam antibiotics (e.g., cephalosporins) due to potential cross-reactivity.
Drug Interactions
Methotrexate
Penicillin V may decrease renal clearance of methotrexate, potentially increasing its toxicity.
Tetracyclines
May antagonize the bactericidal effect of penicillin V.
Oral anticoagulants (e.g., Warfarin)
May enhance the anticoagulant effect by altering gut flora and reducing Vitamin K synthesis.
Live bacterial vaccines (e.g., typhoid vaccine)
Antibiotics may reduce the therapeutic effect of live bacterial vaccines.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, abdominal pain, and rarely neurotoxicity (e.g., seizures) particularly in patients with severe renal impairment. Treatment is supportive and symptomatic; hemodialysis may be effective in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown a risk to the fetus, but there are no adequate and well-controlled studies in pregnant women. Phenoxymethylpenicillin is excreted into breast milk in small amounts; generally considered compatible with breastfeeding, but monitor for rash or diarrhea in the infant.
