Overview of the medicine
Danflex 25 mg Capsule contains Diclofenac Sodium, a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, inflammation, and stiffness caused by various conditions such as arthritis, acute gout, and muscle aches.
Uses & Indications
Dosage
Adults
Usually 25-50 mg, 2-3 times daily, not exceeding 150 mg per day. The lowest effective dose for the shortest duration should be used.
Elderly
Lower initial doses are recommended, with careful monitoring due to increased risk of side effects.
Renal_impairment
Use with caution and at reduced doses. Monitor renal function closely. Avoid in severe renal impairment.
How to Take
Take the capsule whole with a glass of water, preferably with or after food, to minimize gastrointestinal upset. Do not chew or crush the capsule.
Mechanism of Action
Diclofenac Sodium works by inhibiting cyclo-oxygenase (COX) enzymes, COX-1 and COX-2, which are involved in the synthesis of prostaglandins. Prostaglandins are chemicals in the body that cause inflammation, pain, and fever. By reducing prostaglandin production, diclofenac helps to alleviate these symptoms.

Onset
Pain relief usually starts within 30-60 minutes.
Excretion
Approximately 60% is excreted in the urine as metabolites, and about 30-40% is excreted in bile as metabolites.
Half life
Plasma elimination half-life is approximately 1-2 hours.
Absorption
Rapidly and completely absorbed after oral administration. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
Extensively metabolized in the liver, primarily via hydroxylation and glucuronidation, mainly by CYP2C9.

Contraindications
- Hypersensitivity to diclofenac or any excipients
- History of asthma, urticaria, or other allergic-type reactions after taking aspirin or other NSAIDs
- Active peptic ulcer, gastrointestinal bleeding or perforation
- Severe renal or hepatic impairment
- Severe heart failure
- Third trimester of pregnancy
- History of cerebrovascular bleeding or other active bleeding

Lithium and Methotrexate
Increased plasma levels of lithium and methotrexate, leading to toxicity.
Diuretics and ACE inhibitors
Reduced effectiveness of these medications; increased risk of renal impairment.

Overdose
Symptoms of overdose may include drowsiness, lethargy, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. In rare cases, hypertension, acute renal failure, respiratory depression, and coma may occur. Treatment is supportive and symptomatic; there is no specific antidote. Gastric lavage and activated charcoal may be considered if ingested recently.
Pregnancy & Lactation
Pregnancy Category C in early pregnancy, changing to Category D in the third trimester. Avoid during the third trimester due to potential premature closure of the fetal ductus arteriosus. Use with caution during lactation; diclofenac passes into breast milk in small amounts.
