Overview of the medicine
Delpino 75 mg Tablet contains Clopidogrel, an antiplatelet medication used to prevent blood clots in patients with heart conditions, stroke, or peripheral arterial disease.
Uses & Indications
Dosage
Adults
75 mg orally once daily. For acute coronary syndrome, a loading dose of 300 mg (or 600 mg) may be given, followed by 75 mg once daily.
Elderly
No dose adjustment generally required for elderly patients.
Renal_impairment
No dose adjustment is needed for mild-to-moderate renal impairment; caution is advised in severe renal impairment due to limited experience.
How to Take
Administer orally, with or without food. Swallow the tablet whole with a glass of water.
Mechanism of Action
Clopidogrel selectively and irreversibly inhibits the binding of adenosine diphosphate (ADP) to its platelet receptor (P2Y12), thereby preventing ADP-mediated activation of the glycoprotein GPIIb/IIIa complex and subsequent platelet aggregation.
Pharmacokinetics
Onset
Within 2 hours (single dose); maximal effect after 3-7 days of repeated dosing.
Excretion
Approximately 50% via urine and 46% via feces within 120 hours of administration.
Half life
Parent drug: Approximately 8 hours; Active metabolite: Approximately 30 minutes.
Absorption
Rapidly absorbed from the gastrointestinal tract. Absolute bioavailability is approximately 50%.
Metabolism
Extensively metabolized in the liver by CYP450 enzymes (mainly CYP2C19, CYP3A4, CYP2B6, CYP1A2) to an active thiol metabolite and inactive carboxylic acid derivatives.
Side Effects
Contraindications
- •Hypersensitivity to clopidogrel or any component of the formulation.
- •Active pathological bleeding, such as peptic ulcer or intracranial hemorrhage.
- •Severe hepatic impairment.
Drug Interactions
Warfarin
Increased risk of bleeding due to additive effects on hemostasis.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
Increased risk of gastrointestinal bleeding.
Proton Pump Inhibitors (e.g., Omeprazole, Esomeprazole)
May reduce the antiplatelet effect of clopidogrel by inhibiting CYP2C19, potentially increasing the risk of cardiovascular events.
SSRIs (Selective Serotonin Reuptake Inhibitors)/SNRIs (Serotonin-Norepinephrine Reuptake Inhibitors)
Increased risk of bleeding.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Overdose may lead to prolonged bleeding time and subsequent bleeding complications. Management should include symptomatic treatment and measures to manage bleeding, such as platelet transfusion if necessary.
Pregnancy & Lactation
Pregnancy Category B. Animal studies have not shown harm, but human data are limited. Use only if clearly needed. It is unknown if clopidogrel is excreted in human milk; caution should be exercised when administered to a nursing woman.
