Demacro 200 mg/5 ml Suspension contains Azithromycin, a macrolide antibiotic used to treat various bacterial infections in children, including respiratory tract infections, skin and soft tissue infections, and otitis media.
Uses & Indications
Dosage
Children_general
10 mg/kg once daily for 3 days, or 10 mg/kg on Day 1 followed by 5 mg/kg on Days 2-5.
Renal_impairment
No dosage adjustment necessary for mild to moderate renal impairment. Caution with severe impairment.
Hepatic_impairment
No dosage adjustment necessary for mild to moderate hepatic impairment. Use with caution in severe hepatic dysfunction.
Children_otitis_media
30 mg/kg given as a single dose, or 10 mg/kg once daily for 3 days.
How to Take
Shake the bottle well before each use. Administer orally using the provided measuring spoon or syringe. May be taken with or without food. Complete the full prescribed course of treatment.
Mechanism of Action
Azithromycin works by binding to the 50S ribosomal subunit of susceptible microorganisms, thereby inhibiting bacterial protein synthesis. This results in its bacteriostatic effect.

Onset
Antibacterial effects begin within hours of administration.
Excretion
Excreted mainly unchanged in bile. A small percentage is excreted in urine.
Half life
Plasma elimination half-life is approximately 68 hours, allowing for once-daily dosing and shorter treatment courses.
Absorption
Rapidly absorbed from the gastrointestinal tract; oral bioavailability is approximately 37%. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
Primarily metabolized in the liver, with limited first-pass metabolism. No significant interaction with the cytochrome P450 system.
Side Effects
Contraindications
- •Hypersensitivity to azithromycin, erythromycin, or any macrolide or ketolide antibiotic.
- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
Increased digoxin levels. Monitor digoxin levels.
Antacids
Concurrent use may decrease peak serum concentrations of azithromycin. Administer azithromycin at least 1 hour before or 2 hours after antacids.
Warfarin
Increased anticoagulant effect may occur. Monitor prothrombin time/INR.
Nelfinavir
Increased azithromycin serum concentrations. No dosage adjustment needed for azithromycin.
Storage
Store in a cool, dry place below 30°C. Protect from direct light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves symptomatic and supportive measures. Gastric lavage and general supportive care may be indicated.
Pregnancy & Lactation
Pregnancy Category B. Azithromycin should be used during pregnancy only if clearly needed. It is excreted in human milk; use with caution in nursing mothers.
