Overview of the medicine
Demoxiclave is a combination antibiotic containing amoxicillin, a penicillin-type antibiotic, and clavulanic acid, a beta-lactamase inhibitor. It is used to treat a wide range of bacterial infections that are resistant to amoxicillin alone.
Uses & Indications
Dosage
Adults
One 250 mg tablet three times daily, or as directed by physician. For more severe infections, 500 mg three times daily.
Elderly
Dosage should be adjusted based on renal function.
Renal_impairment
Reduced dosage intervals or lower doses may be necessary depending on creatinine clearance.
How to Take
Orally, preferably at the start of a meal to minimize potential gastrointestinal intolerance.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis. Clavulanic acid inhibits beta-lactamase enzymes produced by bacteria, which would otherwise inactivate amoxicillin, thereby extending amoxicillin's spectrum of activity.

Primarily excreted unchanged in the urine by glomerular filtration and tubular secretion.

Approximately 1 hour for amoxicillin, and 1 hour for clavulanic acid.

Metabolism
Both components are partially metabolized in the liver; amoxicillin to penicilloic acid, clavulanic acid to inactive metabolites.

Contraindications
- History of hypersensitivity to penicillin or other beta-lactam antibiotics
- History of cholestatic jaundice/hepatic dysfunction associated with amoxicillin/clavulanic acid
- Severe renal impairment (without dosage adjustment)
Drug Interactions
Warfarin
May prolong prothrombin time; monitor INR.
Probenecid
Increases amoxicillin levels by decreasing renal tubular secretion.
Methotrexate
May increase methotrexate toxicity due to reduced renal tubular secretion.
Oral contraceptives
May reduce efficacy of oral contraceptives.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include gastrointestinal upset and fluid/electrolyte imbalance. Treatment is symptomatic and supportive. Hemodialysis can remove both components from circulation.

Pregnancy Category B. Use with caution during pregnancy if clearly needed. Excreted in breast milk; use with caution during lactation.
