Demoxiclave 875 mg Tablet is a combination antibiotic containing amoxicillin and clavulanate potassium. Amoxicillin is a penicillin-type antibiotic that fights bacteria, while clavulanate potassium prevents bacteria from destroying amoxicillin. It is used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
One 875 mg/125 mg tablet every 12 hours for 7-10 days, depending on the infection.
Elderly
No specific dose adjustment needed unless renal function is impaired. Monitor renal function.
Renal_impairment
Dosage adjustment required based on creatinine clearance. For CrCl 10-30 mL/min: 875 mg/125 mg every 24 hours. For CrCl <10 mL/min: Not recommended for 875/125 mg formulation, consider lower strength or alternative.
How to Take
Take the tablet orally at the start of a meal to minimize potential gastrointestinal intolerance. Swallow whole with a glass of water.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death. Clavulanate potassium is a beta-lactamase inhibitor that irreversibly binds to and inactivates beta-lactamase enzymes produced by some bacteria, protecting amoxicillin from degradation and extending its antibacterial spectrum.

Excretion
Primarily excreted unchanged by the kidneys via glomerular filtration and tubular secretion. Clavulanate is excreted renally and non-renally.
Half life
Amoxicillin: 1-1.5 hours; Clavulanate: 1 hour.
Absorption
Both amoxicillin and clavulanate potassium are well absorbed from the gastrointestinal tract after oral administration. Absorption is not significantly affected by food.
Metabolism
Amoxicillin is partially metabolized; clavulanate is extensively metabolized.
Side Effects
Contraindications
- •History of severe hypersensitivity reaction to beta-lactam antibiotics (e.g., anaphylaxis to penicillin or cephalosporin)
- •History of amoxicillin/clavulanate-associated cholestatic jaundice or hepatic dysfunction
Drug Interactions
Warfarin
May prolong prothrombin time; monitor INR closely.
Probenecid
Increases amoxicillin blood levels by decreasing renal tubular secretion (but not clavulanate).
Allopurinol
Increased incidence of rash.
Methotrexate
May increase methotrexate toxicity by reducing its renal excretion.
Oral contraceptives
May reduce efficacy of oral contraceptives (though evidence is weak).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include gastrointestinal upset and electrolyte imbalance. Rarely, renal impairment or convulsions. Treatment is symptomatic and supportive; hemodialysis can remove amoxicillin and clavulanate from circulation.
Pregnancy & Lactation
Pregnancy Category B. Generally considered safe. Use during lactation is generally considered safe, but monitor infant for diarrhea or candidiasis.
