Description
Overview of the medicine
Dermastin 0.05% w/w Cream contains Clobetasol Propionate, a highly potent corticosteroid, used for short-term treatment of inflammatory skin conditions unresponsive to less potent corticosteroids.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area once or twice daily for a maximum of two weeks. Do not use more than 50g per week.
Elderly
Use with caution and for the shortest possible duration, as skin thinning may be more pronounced.
Renal_impairment
No specific dosage adjustment required, but systemic absorption should be monitored if large areas are treated.
How to Take
For topical dermatological use only. Apply a small amount of cream to the affected skin and rub gently until it disappears. Wash hands after application unless hands are the treated area.
Mechanism of Action
Clobetasol Propionate is a topical corticosteroid that acts by inducing phospholipase A2 inhibitory proteins, lipocortins. These proteins control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes by inhibiting the release of their common precursor, arachidonic acid.
Pharmacokinetics
Onset
Within hours of application.
Excretion
Excreted mainly via the kidneys, with some excretion in bile.
Half life
Not well established for topical application; systemic half-life of absorbed clobetasol is about 3-5 hours.
Absorption
Minimal systemic absorption occurs through intact skin; absorption increases with skin damage, occlusion, or application to large surface areas.
Metabolism
Metabolized primarily in the liver to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Clobetasol Propionate or any components of the cream
- Rosacea, perioral dermatitis, acne vulgaris
- Pruritus without inflammation
- Perianal and genital pruritus
- Primary cutaneous viral infections (e.g., herpes simplex, chickenpox)
- Primary bacterial or fungal skin infections
- Children under 1 year of age
Drug Interactions
CYP3A4 Inhibitors (e.g., Ritonavir, Itraconazole)
Co-administration with potent CYP3A4 inhibitors may increase systemic exposure to clobetasol, leading to an increased risk of systemic side effects. Use with caution.
Storage
Store below 30°C. Do not freeze. Protect from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is unlikely with topical application. Prolonged or excessive use may lead to systemic effects such as Cushing's syndrome, hyperglycemia, and HPA axis suppression. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus, and under strict medical supervision. Avoid use on large areas or for prolonged periods. Consult a doctor before use during breastfeeding.
